Washington superbill and invoice for LMHC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Washington selected. Out-of-network clients still need a superbill with your NPI, tax ID, Washington license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Washington superbills
Independent counseling in Washington is commonly written as LMHC. Also print the letters you actually hold: LMHC, LICSW, LMFT, Licensed Psychologist, LMHCA. The board to match your number against is the Washington State Department of Health. License line hint: WA LH #.
Washington counselors are LMHCs. Independent social workers use LICSW. License numbers often start with a profession prefix (LH).
Telehealth coding (national rules, used in Washington)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Washington
Washington generally does not charge retail sales tax on professional healthcare counseling; confirm if you sell non-clinical products or trainings. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
What is an LH number?
DOH credential numbers are prefixed. Copy the full credential string from the Provider Credential Search.
What to put on a Washington superbill
- Your legal name, LMHC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Washington license number as printed by the Washington State Department of Health
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Washington counselors are Licensed Mental Health Counselors (LMHC), not LPCs. Credentials are issued by the Washington State Department of Health (DOH). This generator builds a therapist invoice or out-of-network (OON) superbill PDF in the browser with Washington defaults: LMHC as the primary title, LMHCA for associates, LICSW for independent social workers, and a WA LH # license hint matching DOH’s profession-prefixed credential strings.
An invoice bills the client for payment. A superbill packages CPT, ICD-10, provider, and payment detail for OON reimbursement. Both need accurate name, credential, license number, NPI, diagnoses, procedures, place of service, and amounts. Processing is browser-only—no PHI upload for this tool.
License titles that belong on the form
Washington’s independent counseling license is LMHC. Associates hold LMHCA. Putting “LPC” on a Washington superbill looks like an out-of-state credential.
Independent titles commonly used on Washington superbills:
- LMHC — Licensed Mental Health Counselor (default on this page)
- LICSW — Licensed Independent Clinical Social Worker
- LMFT — Licensed Marriage and Family Therapist
- Licensed Psychologist — Washington examining board for psychology (DOH credential)
Associate / pre-independent:
- LMHCA — Licensed Mental Health Counselor Associate
Disclose associate status and supervisor when plans ask. Do not print LMHC until DOH has issued the independent credential.
Exact strings for this generator’s credential dropdown: `LMHC`, `LICSW`, `LMFT`, `Licensed Psychologist`, `LMHCA`.
Board, license number, lookup
Agency: Washington State Department of Health — Mental Health Counselor licensing pages at doh.wa.gov.
Official credential search: Provider Credential or Facility Search.
DOH credential numbers are often profession-prefixed (mental health counselor credentials commonly use an LH prefix). Copy the full credential string from Provider Credential Search—not a truncated number. High-volume verification can also use Washington’s Open Data Portal; do not scrape the interactive search tools.
Interstate practice: Counseling Compact / PSYPACT / Social Work Compact
As of retrieval on 2026-09-14:
- Counseling Compact: Washington is a member (enacted) on counselingcompact.gov/map. Confirm operational privilege status before CompactConnect use—enacted is not automatically live.
- PSYPACT: Washington is a participating jurisdiction for psychologists. Verify psypact.gov and hold current authorization.
- Social Work Licensure Compact: Washington has enacted the compact and appears on swcompact.org/compact-jurisdictions. Multistate SW licenses were not yet issuing as of mid-2026—confirm before marketing.
Verify before you rely on any compact.
Telehealth + place of service
For video sessions, commercial superbills commonly use:
- POS 10 — telehealth in patient’s home
- POS 02 — telehealth other than patient’s home
- Modifier 95 — synchronous audio-video telemedicine (when required)
Do not use POS 11 for pure video. Print the Washington credential (or compact privilege documentation payers accept) that authorizes practice where the client is located.
Sales tax on therapy invoices
Washington expanded retail sales tax to additional service categories under ESSB 5814 (effective October 1, 2025). DOR interim guidance states that the Legislature did not intend ordinary professional services to become retail sales tax solely because they are delivered using digital tools, and professional services were not newly added to the retail-sale definition in RCW 82.04.050 (DOR interim guidance on ESSB 5814). Core licensed mental health counseling fees generally remain under Service and Other Activities B&O, not retail sales tax—leave the sales-tax line at $0 on session invoices unless you sell taxable goods, live presentations/trainings, digital automated services, or other newly enumerated items. Confirm with DOR or a tax professional for hybrid offerings.
This is not tax advice. B&O tax reporting is separate from the invoice sales-tax line.

Two worked Washington examples
Washington's paperwork runs through the Department of Health rather than a licensing board, and its credential alphabet is the Northwest pattern: LMHC for counselors, LICSW for clinical social workers, LMHCA for associates — with DOH credential numbers commonly carrying a profession prefix like LH. The two documents below come from a fictional Spokane practice. All identifiers are placeholders.
Example 1 — an in-office private-pay Washington invoice
An LMHC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Jun 3 | Individual psychotherapy, 45 min | 90834 | 11 | $165.00 | $165.00 |
| Jun 10 | Individual psychotherapy, 45 min | 90834 | 11 | $165.00 | $165.00 |
| Jun 17 | Individual psychotherapy, 45 min | 90834 | 11 | $165.00 | $165.00 |
| Jun 24 | Individual psychotherapy, 45 min | 90834 | 11 | $165.00 | $165.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $660.00 | $660.00 | |||
POS 11 is correct for office sessions. The tax line stays at $0.00 because Washington generally does not charge retail sales tax on professional healthcare counseling — DOR's interim guidance after ESSB 5814 confirms professional services were not newly added to the retail-sale definition, so counseling receipts typically remain under Service & Other Activities B&O rather than retailing. The credential is LMHC, and the license field follows the WA LH # hint — copy the full profession-prefixed credential string from Provider Credential Search.
Example 2 — a telehealth out-of-network superbill
A second client in Tacoma has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Sep 2 | 90791 | 95 | 10 | F41.1 | $195.00 | $195.00 |
| Sep 9 | 90834 | 95 | 10 | F41.1 | $165.00 | $165.00 |
| Sep 16 | 90834 | 95 | 10 | F41.1 | $165.00 | $165.00 |
| Sep 23 | 90834 | 95 | 10 | F41.1 | $165.00 | $165.00 |
| Total | $690.00 | $690.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous audio-video service for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LMHC, the LH-prefixed credential number copied verbatim from DOH's Provider Credential Search, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures.
Before the document goes out, run the credential string through Provider Credential Search and confirm three things agree: the name, the LMHC credential type, and an active status. If the rendering clinician is an independent clinical social worker rather than a counselor, the credential is LICSW under a different DOH credential series — verify it in the same search and print the letters as shown. In a group practice, the superbill lists the individual rendering clinician while the tax ID stays the practice's; when the plan asks for a supervising clinician for associate-rendered care, add the supervisor's name and credential in the notes rather than in the signature block.
Both parties were in Washington, so the state credential sufficed. Washington has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations. An LMHCA delivering these sessions would print the associate credential and identify the supervisor when the plan asks.
Common mistakes on Washington superbills
- Writing "LPC" instead of "LMHC". Washington's independent counseling license is Licensed Mental Health Counselor — LPC reads as an out-of-state credential and fails DOH verification.
- Printing LMHC while holding LMHCA. The Licensed Mental Health Counselor Associate credential is the supervised tier. Disclose LMHCA and identify the supervisor when the plan asks.
- Truncating the credential number. DOH credential numbers are profession-prefixed — mental health counselors commonly carry an LH prefix. Copy the full string from Provider Credential Search, not a stripped digit sequence.
- Misreading the ESSB 5814 headlines. Washington's 2025 tax expansion did not pull professional counseling into retail sales tax — DOR interim guidance keeps it under Service & Other Activities B&O. Leave the sales-tax line at $0, but confirm if you sell trainings or digital products.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments gives the adjuster nothing to act on.
FAQ
What is an LH number?
DOH credential numbers are prefixed. Copy the full credential string from Provider Credential Search.
LPC or LMHC in Washington?
LMHC. “LPC” looks out-of-state to payer processors.
Does Washington charge sales tax on therapy?
Core professional healthcare counseling generally is not retail sales tax; leave tax at $0 on session invoices. Confirm if you sell products, webinars, or digital tools—ESSB 5814 expanded other service categories.
Is Washington in the Counseling Compact?
Enacted member—confirm operational status on counselingcompact.gov before relying on privileges.
What do OON payers want?
Name + LMHC (or other WA title), full DOH credential #, NPI, DOS, CPT, ICD-10, POS, charges, amount paid, telehealth modifier when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Washington State Department of Health, the official interstate-compact commissions, and Washington's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
