Alabama superbill and invoice for LPC, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Alabama selected. Out-of-network clients still need a superbill with your NPI, tax ID, Alabama license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Alabama superbills

Independent counseling in Alabama is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Alabama Board of Examiners in Counseling / ASBSWE. License line hint: AL license #.

Alabama uses LPC (and ALC for associates). Put the full credential after your name on the superbill.

Telehealth coding (national rules, used in Alabama)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Alabama

Mental health services are generally not subject to Alabama sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

Can an Alabama ALC issue a superbill?

Associate Licensed Counselors practice under supervision. Many commercial payers expect the rendering provider to be independently licensed. Confirm with the client's plan before issuing an OON superbill under an associate credential.

What to put on a Alabama superbill

  • Your legal name, LPC or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Alabama license number as printed by the Alabama Board of Examiners in Counseling / ASBSWE
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Alabama private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Alabama license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select Alabama defaults when your rendering authority and the client’s location for the session are Alabama (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your Alabama Board of Examiners in Counseling (ABEC) or sister-board record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

Alabama’s independent counseling title is Licensed Professional Counselor (LPC). That is the generator default and the string most commercial payers expect from an Alabama counseling practice. Pre-independent counselors use Associate Licensed Counselor (ALC) under ABEC supervision rules—print ALC, not LPC, while accruing hours toward independent practice.

Other titles commonly used on Alabama superbills:

  • LCSW — Licensed Clinical Social Worker (Alabama State Board of Social Work Examiners / ASBSWE).
  • LMFT — Licensed Marriage and Family Therapist (print the exact letters on your license).
  • Licensed Psychologist — psychologists licensed through the Alabama Board of Examiners in Psychology—print the exact title on your wall certificate.

Exact credential dropdown strings:

`LPC` · `LCSW` · `LMFT` · `Licensed Psychologist`

Independently licensed LPCs list their own license only. ALCs must use the associate title and should identify the supervisor when a payer asks. Do not put “LPC” on the form during the associate period.

Board, license number, lookup

Counselor licenses (LPC / ALC) are issued by the Alabama Board of Examiners in Counseling (ABEC) — abec.alabama.gov. Social work licenses are issued by the Alabama State Board of Social Work Examiners (ASBSWE). Psychologists verify through their own psychology board.

Official LPC/ALC verification: ABEC’s primary-source Licensee Search (linked from abec.alabama.gov/licensees/licensee-search). Copy the license number exactly as shown. The generator hint AL license # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — enacted (not yet operational for privileges). Alabama appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both the home state and the client’s state are live/operational—verify the current live list on counselingcompact.gov before relying on a privilege.
  • PSYPACT — yes. Alabama participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize LPC, LCSW, or LMFT practice.
  • Social Work Licensure Compact — enacted. Alabama is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system—confirm at swcompact.org before advertising multistate SW practice.

Verify before you rely on it. Compact membership and privilege systems change. This page’s flags are for generator defaults only.

Telehealth + place of service

For video psychotherapy superbills, use the place-of-service codes payers expect:

  • POS 10 — patient at home
  • POS 02 — telehealth other than home
  • Modifier 95 — when the plan wants synchronous telemedicine marked

Do not code a video-only session as POS 11. An Alabama license, a live Counseling Compact privilege naming Alabama (when operational), or another lawful authorization must cover the client’s location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

Alabama sales and use tax generally applies to retail sales of tangible personal property and certain enumerated services (for example, amusement, rentals, and fabrication labor). Separately stated repair/install labor and many professional services fall outside the taxable base. Professional counseling and psychotherapy session fees are therefore generally not subject to Alabama sales tax.

Leave the invoice sales tax at $0 for clinical services. If you also sell taxable tangible goods, collect tax on those goods as required. Confirm edge cases with the Alabama Department of Revenue or a tax professional.

Two worked Alabama examples

Alabama splits its behavioral-health licensing across boards — counselors answer to ABEC while clinical social workers answer to ASBSWE — and the credential on the form has to match whichever board issued it. The two documents below come from a fictional Birmingham practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.

Example 1 — an in-office private-pay Alabama invoice

An LPC sees a self-pay client for weekly 45-minute sessions in the office. With no insurer involved, the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
Apr 8Individual psychotherapy, 45 min9083411$120.00$120.00
Apr 15Individual psychotherapy, 45 min9083411$120.00$120.00
Apr 22Individual psychotherapy, 45 min9083411$120.00$120.00
Apr 29Individual psychotherapy, 45 min9083411$120.00$120.00
Sales tax (see below)$0.00
Total$480.00$480.00

POS 11 is correct for office sessions. The tax line reads $0.00 because Alabama's sales tax reaches tangible personal property and enumerated services, and professional counseling session fees fall outside that taxable base. The credential is LPC, Alabama's independent counseling title, and the license field follows the AL license # hint — copy the number exactly as it appears in ABEC's Licensee Search.

Example 2 — a telehealth out-of-network superbill

A second client in Huntsville has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Jul 7907919510F33.1$155.00$155.00
Jul 14908349510F33.1$120.00$120.00
Jul 21908349510F33.1$120.00$120.00
Jul 28908349510F33.1$120.00$120.00
Total$515.00$515.00

The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.

The provider block shows the rendering name with LPC, the ABEC license number copied from the official Licensee Search, NPI, practice address, and tax ID. If the rendering clinician were a social worker instead, the credential would be LCSW and the number would come from ASBSWE's records — Alabama keeps the two on separate boards, and the payer will verify against whichever board issued the credential printed. The paid column is populated because OON reimbursement repays money already spent.

Both parties were in Alabama, so the state license sufficed. Alabama has enacted the Counseling Compact but was not in the early operational privilege-issuing wave — an Alabama LPC reaching a client in another compact state should verify that both states are live before relying on a privilege.

Common mistakes on Alabama superbills

  • Printing LPC while holding ALC. Associate Licensed Counselors practice under ABEC supervision. Print ALC and identify the supervisor when the plan asks — do not borrow the independent title early.
  • Verifying against the wrong board. Counselors verify through ABEC's Licensee Search; clinical social workers through ASBSWE. A payer checking the wrong board sees no record — print the number from the board that issued the credential.
  • Treating enacted compact status as live. Alabama is a Counseling Compact member, but privileges require both states to be operational. Check counselingcompact.gov before relying on one.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 means the client physically sat in your office.
  • Adding a sales-tax line to session fees. Professional counseling is outside Alabama's taxable retail-services pattern. Leave tax at $0 unless you sell taxable goods.
  • Leaving the paid column blank. OON plans reimburse actual expenditures — a superbill without payments gives the adjuster nothing to act on.

FAQ

Can an Alabama ALC issue a superbill?

Associate Licensed Counselors practice under supervision. Many commercial payers expect the rendering provider to be independently licensed. Confirm with the client’s plan before issuing an OON superbill under an associate credential; print ALC (not LPC) and identify the supervisor when asked.

Is Alabama in the Counseling Compact?

Yes—enacted. Privileges work only with other live/operational states; check counselingcompact.gov before interstate counseling claims.

Does Alabama tax therapy sessions?

Professional psychotherapy is generally outside Alabama’s taxable retail-services pattern. Leave tax at $0 on session-only invoices unless you sell taxable goods.

Can an Alabama psychologist use PSYPACT?

Alabama participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.

What do OON payers want on an Alabama superbill?

Name + Alabama credential, board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.

Why these sources matter

Every factual claim on this page traces back to a primary source: Alabama Board of Examiners in Counseling (ABEC); Alabama State Board of Social Work Examiners (ASBSWE), the official interstate-compact commissions, and Alabama's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
