Kentucky superbill and invoice for LPCC, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Kentucky selected. Out-of-network clients still need a superbill with your NPI, tax ID, Kentucky license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Kentucky superbills

Independent counseling in Kentucky is commonly written as LPCC. Also print the letters you actually hold: LPCA, LPCC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Kentucky Board of Licensed Professional Counselors. License line hint: KY LPCC #.

Kentucky's independent counseling title is LPCC (LPCA while accruing hours).

Telehealth coding (national rules, used in Kentucky)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Kentucky

Professional counseling services are generally exempt from Kentucky sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

Is Kentucky in the Counseling Compact?

Kentucky has enacted the compact. Privileges are only usable when both the home state and the client's state are operational — check counselingcompact.gov before relying on it.

What to put on a Kentucky superbill

  • Your legal name, LPCC or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Kentucky license number as printed by the Kentucky Board of Licensed Professional Counselors
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Kentucky private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Kentucky license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select Kentucky defaults when your rendering authority and the client’s location for the session are Kentucky (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your Kentucky Board of Licensed Professional Counselors record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

Kentucky’s independent clinical counseling title is Licensed Professional Clinical Counselor (LPCC). That is the generator default. Pre-independent counselors use Licensed Professional Counselor Associate (LPCA) while accruing hours—print LPCA, not LPCC, during the associate period.

Other titles commonly used on Kentucky superbills:

  • LCSW — Licensed Clinical Social Worker (print the exact letters on your license).
  • LMFT — Licensed Marriage and Family Therapist (Kentucky Board of Licensure for Marriage and Family Therapists).
  • Licensed Psychologist — psychologists licensed by the Kentucky Board of Examiners of Psychology—print the exact title on your license.

Exact credential dropdown strings:

`LPCA` · `LPCC` · `LCSW` · `LMFT` · `Licensed Psychologist`

Do not put “LPCC” on the form if you only hold LPCA. LPCAs should identify the supervisor when a payer asks. LPCC-S (supervisor designation) may be noted in the notes field; the credential line should still show LPCC when that is the license held.

Board, license number, lookup

Counselor licenses (LPCA / LPCC) are issued by the Kentucky Board of Licensed Professional Counselors under the Department of Professional Licensing — lpc.ky.gov.

Official verification: Kentucky’s online license search—select the Board of Licensed Professional Counselors, then search by name or license number. Copy the number exactly as shown. The generator hint KY LPCC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus tax ID as the payer requires.

MFT and psychology licenses verify through their respective Kentucky boards on the same DPL search portal.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — enacted (not yet operational for privileges). Kentucky appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
  • PSYPACT — yes. Kentucky participates for psychologists with required ASPPB authorizations. Confirm on psypact.gov. PSYPACT does not authorize LPCC/LPCA, LCSW, or LMFT practice.
  • Social Work Licensure Compact — enacted. Kentucky is listed on swcompact.org/compact-jurisdictions. Multistate SW licenses are not yet issuing—confirm at swcompact.org.

Verify before you rely on it. Compact membership and privilege systems change.

Telehealth + place of service

Use POS 10 when the patient is at home, or POS 02 for telehealth elsewhere. Add modifier 95 for synchronous telemedicine when the payer’s guide requires it. Do not use POS 11 for a video-only visit.

A Kentucky LPCC (or LPCA under supervision), a live Counseling Compact privilege naming Kentucky (when operational), or another lawful authorization must cover the client’s location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

Kentucky’s 103 KAR 26:010 classifies persons rendering nontaxable professional services (examples include doctors, lawyers, accountants, and engineers) as consumers, not retailers, of incidental supplies. Professional counseling and psychotherapy session fees are generally treated as nontaxable professional services—not among the services specifically enumerated as taxable in KRS 139.200.

Leave the invoice sales tax at $0 for clinical services. If you also regularly sell tangible personal property or enumerated taxable services, remit tax on those sales. Confirm edge cases with the Kentucky Department of Revenue or a tax professional.

How to fill the Kentucky generator

  1. Choose Superbill or Invoice.
  2. Set jurisdiction to KY.
  3. Enter rendering name; select LPCC (default) or LPCA / LCSW / LMFT / Licensed Psychologist as held.
  4. Complete practice demographics, NPI, tax ID, and Kentucky board license number.
  5. Add client member ID / DOB only if needed for the OON claim.
  6. Enter DOS, CPT, ICD-10, units, fees, and payments.
  7. Telehealth: POS 10 or 02; modifier 95 when required.
  8. Keep sales tax $0 on therapy fees.
  9. Download the PDF locally—nothing is uploaded.

Two worked Kentucky examples

Kentucky's paperwork turns on a credential distinction that trips up clinicians who trained elsewhere: LPCC here is the independent clinical title, while LPCA is the supervised associate tier — and the LPCC-S supervisor designation belongs in notes, not on the credential line. The two documents below come from a fictional Louisville practice. All identifiers are placeholders.

Example 1 — an in-office private-pay Kentucky invoice

An LPCC sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
Apr 7Individual psychotherapy, 60 min9083711$140.00$140.00
Apr 14Individual psychotherapy, 60 min9083711$140.00$140.00
Apr 21Individual psychotherapy, 60 min9083711$140.00$140.00
Apr 28Individual psychotherapy, 60 min9083711$140.00$140.00
Sales tax (see below)$0.00
Total$560.00$560.00

POS 11 is correct for office sessions. The tax line reads $0.00 because Kentucky classifies nontaxable professional service providers as consumers under 103 KAR 26:010 — professional counseling session fees are not a taxable retail line. The credential is LPCC, Kentucky's independent clinical counseling title, and the license field follows the KY LPCC # hint — copy the number exactly as the Department of Professional Licensing's online search displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Lexington has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Sep 8907919510F41.1$165.00$165.00
Sep 15908379510F41.1$140.00$140.00
Sep 22908379510F41.1$140.00$140.00
Sep 29908379510F41.1$140.00$140.00
Total$585.00$585.00

The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous telemedicine for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.

The provider block carries the rendering name with LPCC, the license number copied verbatim from the oop.ky.gov license search — selecting the Board of Licensed Professional Counselors first — plus NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays money already spent.

Both parties were in Kentucky, so the state license sufficed. Kentucky has enacted the Counseling Compact — privileges require both ends to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations. An LPCA delivering these sessions would print the associate credential and identify the supervisor when the plan asks; the associate tier is not a compact pathway.

Common mistakes on Kentucky superbills

  • Printing LPCC while holding LPCA. The Licensed Professional Counselor Associate credential is the supervised tier. Disclose LPCA and identify the supervisor when the plan asks.
  • Putting LPCC-S on the credential line. The supervisor designation is noted in the notes field; the credential line still shows LPCC when that is the license held.
  • Searching the wrong board. Kentucky's DPL search covers multiple boards — select the Board of Licensed Professional Counselors before copying the number, and use the MFT or psychology board's record for those credentials.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Adding a sales-tax line. Professional services are nontaxable under the 103 KAR 26:010 consumer rule. Leave tax at $0 unless you sell taxable goods or enumerated services.
  • Leaving the paid column blank. OON plans reimburse actual expenditures — a superbill without payments usually comes back unanswered.

FAQ

Is Kentucky in the Counseling Compact?

Yes—enacted. Privileges are only usable when both the home state and the client’s state are operational—check counselingcompact.gov before relying on it.

Can an LPCA issue an OON superbill?

Disclose the associate title and supervisor. Many commercial plans still process OON superbills if the rendering line is accurate—confirm with the plan first. Do not print LPCC while holding only LPCA.

Does Kentucky tax therapy sessions?

Professional counseling services are generally nontaxable under 103 KAR 26:010 / KRS 139.200 patterns. Leave tax at $0 on session-only invoices unless you sell taxable goods or enumerated services.

Can a Kentucky psychologist use PSYPACT?

Kentucky participates in PSYPACT. You still need active authorizations—verify on psypact.gov.

What do OON payers want on a Kentucky superbill?

Name + Kentucky credential (LPCC or LPCA as held), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.

Why these sources matter

Every factual claim on this page traces back to a primary source: Kentucky Board of Licensed Professional Counselors, the official interstate-compact commissions, and Kentucky's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
