Arkansas superbill and invoice for LPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Arkansas selected. Out-of-network clients still need a superbill with your NPI, tax ID, Arkansas license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Arkansas superbills
Independent counseling in Arkansas is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Arkansas Board of Examiners in Counseling and MFT. License line hint: AR LPC #.
Arkansas uses LPC and LAC. Counseling Compact privileges have begun rolling out; confirm both home and remote states are operational before relying on the compact.
Telehealth coding (national rules, used in Arkansas)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Arkansas
Therapy services are generally exempt from Arkansas sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Operational / issuing privileges
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Should I list my LAC or LPC?
Use the credential you currently hold. If you are an LAC, include the supervisor's name and license in the notes field when a payer asks for it.
What to put on a Arkansas superbill
- Your legal name, LPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Arkansas license number as printed by the Arkansas Board of Examiners in Counseling and MFT
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Arkansas private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Arkansas license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select Arkansas defaults when your rendering authority and the client's location for the session are Arkansas (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
Arkansas's independent counseling title is Licensed Professional Counselor (LPC). Pre-independent counselors use Licensed Associate Counselor (LAC) under board supervision—print LAC, not LPC, while accruing hours.
Other titles commonly used on Arkansas superbills:
- LAC — print the exact letters on your Arkansas license/board record.
- LCSW — print the exact letters on your Arkansas license/board record.
- LMFT — print the exact letters on your Arkansas license/board record.
- Licensed Psychologist — print the exact letters on your Arkansas license/board record.
Exact credential dropdown strings:
`LPC` · `LAC` · `LCSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Counselor and marriage-and-family licenses are issued by the Arkansas Board of Examiners in Counseling. Use the official Arkansas.gov Counselor and Marriage-Family Therapist Licensee Search for primary-source verification.
Official verification: https://portal.arkansas.gov/service/ar-counselor-mariage-fam-therapist-lic-search/. Copy the license number exactly as shown. The generator hint AR LPC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — operational. Arkansas is among the live jurisdictions issuing privileges (counselingcompact.gov lists Arkansas, Arizona, Georgia, Indiana, Louisiana, Minnesota, Ohio, Tennessee, and Wyoming as live as of this retrieval). Privileges require both home and remote states to be live.
- PSYPACT — yes. Arkansas participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
- Social Work Licensure Compact — enacted. Arkansas is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A Arkansas license, a live Counseling Compact privilege naming Arkansas (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Arkansas sales and use tax generally applies to retail sales of tangible personal property and certain enumerated services. Professional counseling and psychotherapy session fees are generally exempt. Leave sales tax at $0 on clinical invoices; collect tax on taxable goods if you sell them. Confirm with Arkansas DFA or a tax professional.

Two worked Arkansas examples
Arkansas sits in the first wave of live Counseling Compact states — one of the few places where "operational" is the accurate word — which changes what interstate telehealth can lawfully look like. The two documents below come from a fictional Little Rock practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Arkansas invoice
An LPC sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — a record of services and payments — with no diagnosis column required.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Jun 4 | Individual psychotherapy, 60 min | 90837 | 11 | $135.00 | $135.00 |
| Jun 11 | Individual psychotherapy, 60 min | 90837 | 11 | $135.00 | $135.00 |
| Jun 18 | Individual psychotherapy, 60 min | 90837 | 11 | $135.00 | $135.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $405.00 | $405.00 | |||
POS 11 is accurate for office work. The tax line stays at $0.00 because professional counseling and psychotherapy session fees are generally exempt from Arkansas sales and use tax, which targets tangible property and enumerated services. The credential is LPC, and the license field follows the AR LPC # hint — copy the number exactly as it appears in the Arkansas.gov counselor and marriage-and-family therapist licensee search.
Example 2 — a telehealth out-of-network superbill
A second client in Fayetteville has a PPO and attends weekly by video from home. The document must satisfy an adjuster, so it becomes a superbill: the invoice fields plus diagnosis, telehealth place of service, the telemedicine modifier, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Aug 6 | 90791 | 95 | 10 | F32.1 | $165.00 | $165.00 |
| Aug 13 | 90837 | 95 | 10 | F32.1 | $135.00 | $135.00 |
| Aug 20 | 90837 | 95 | 10 | F32.1 | $135.00 | $135.00 |
| Aug 27 | 90837 | 95 | 10 | F32.1 | $135.00 | $135.00 |
| Total | $570.00 | $570.00 | ||||
The client was at home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous audio-video service for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.
The provider block carries the rendering name with LPC, the board license number copied verbatim from the portal.arkansas.gov licensee search, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures — a blank paid column usually returns the document unanswered.
Both parties were in Arkansas, so the state license sufficed. Arkansas is genuinely operational for Counseling Compact privileges — one of the early live jurisdictions alongside Arizona, Georgia, Indiana, Louisiana, Minnesota, Ohio, Tennessee, and Wyoming — so an Arkansas LPC can hold privileges in other live compact states and counselors from live states can hold privileges for Arkansas clients. The privilege still has to name the remote state, and both states must be live on the date of service.
Common mistakes on Arkansas superbills
- Printing LPC while holding LAC. Licensed Associate Counselors practice under board supervision while accruing hours. Print LAC and identify the supervisor when the plan asks — do not borrow the independent title early.
- Assuming the compact works one direction only. Arkansas's operational status cuts both ways — out-of-state counselors with live privileges can treat Arkansas clients, and Arkansas LPCs can hold privileges elsewhere. Verify both ends on counselingcompact.gov.
- Guessing the license number. Copy the exact string from the Arkansas.gov licensee search rather than typing from memory — a mistyped number fails the payer's verification.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 is for in-person office services only.
- Adding a sales-tax line. Therapy session fees are generally exempt from Arkansas sales tax. Leave the line at $0 unless you sell taxable goods.
- Leaving the paid column blank. OON reimbursement repays money already spent. Show amounts paid, not just charges.
FAQ
Should I list my LAC or LPC?
Use the credential you currently hold. If you are an LAC, include the supervisor's name and license in the notes field when a payer asks for it.
Is Arkansas in the Counseling Compact?
Yes—operational. Privileges work only with other live states; check counselingcompact.gov before interstate counseling claims.
Does Arkansas tax psychotherapy invoices?
Professional therapy session fees are generally exempt from Arkansas sales tax. Leave tax at $0 on session-only invoices unless you sell taxable goods.
Can a Arkansas psychologist use PSYPACT?
Arkansas participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.
What do OON payers want on a Arkansas superbill?
Name + Arkansas credential (LPC or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Arkansas Board of Examiners in Counseling, the official interstate-compact commissions, and Arkansas's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
