Nebraska superbill and invoice for LIMHP, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Nebraska selected. Out-of-network clients still need a superbill with your NPI, tax ID, Nebraska license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Nebraska superbills

Independent counseling in Nebraska is commonly written as LIMHP. Also print the letters you actually hold: LMHP, LIMHP, LICSW, LMFT, Licensed Psychologist. The board to match your number against is the Nebraska DHHS — Licensure Unit. License line hint: NE LIMHP #.

Nebraska uses LMHP and the independent mental health title LIMHP. Most private-practice superbills should show LIMHP if that is the license held.

Telehealth coding (national rules, used in Nebraska)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Nebraska

Professional mental health services are generally exempt from Nebraska sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

LMHP or LIMHP?

LIMHP is the independent practice license. Print LIMHP when you are independently licensed.

What to put on a Nebraska superbill

  • Your legal name, LIMHP or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Nebraska license number as printed by the Nebraska DHHS — Licensure Unit
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Nebraska private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Nebraska license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select Nebraska defaults when your rendering authority and the client's location for the session are Nebraska (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

Nebraska uses Licensed Mental Health Practitioner (LMHP) and the independent Licensed Independent Mental Health Practitioner (LIMHP). Most private-practice OON superbills should show LIMHP when that is the license held. Independent clinical social workers commonly use LICSW.

Other titles commonly used on Nebraska superbills:

  • LMHP — print the exact letters on your Nebraska license/board record.
  • LICSW — print the exact letters on your Nebraska license/board record.
  • LMFT — print the exact letters on your Nebraska license/board record.
  • Licensed Psychologist — print the exact letters on your Nebraska license/board record.

Exact credential dropdown strings:

`LMHP` · `LIMHP` · `LICSW` · `LMFT` · `Licensed Psychologist`

Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.

Board, license number, lookup

Mental health practice licenses are issued through Nebraska DHHS Licensure. Verify via the official Nebraska license search and copy the number exactly.

Official verification: https://www.nebraska.gov/LISSearch/search.cgi. Copy the license number exactly as shown. The generator hint NE LIMHP # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — enacted (not yet operational for privileges). Nebraska appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
  • PSYPACT — yes. Nebraska participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
  • Social Work Licensure Compact — enacted. Nebraska is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.

Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.

Telehealth + place of service

For video psychotherapy superbills, use the place-of-service codes payers expect:

  • POS 10 — patient at home
  • POS 02 — telehealth other than home
  • Modifier 95 — when the plan wants synchronous telemedicine marked

Do not code a video-only session as POS 11. A Nebraska license, a live Counseling Compact privilege naming Nebraska (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

Nebraska sales tax generally applies to tangible personal property and certain taxable services. Professional mental health counseling/psychotherapy is generally exempt. Leave tax at $0 on clinical invoices. Confirm with the Nebraska Department of Revenue if you sell goods.

How to fill the Nebraska generator

  1. Choose Superbill or Invoice.
  2. Set jurisdiction to NE.
  3. Enter rendering name; select LIMHP (default) or another credential you currently hold from the dropdown.
  4. Complete practice demographics, NPI, tax ID, and Nebraska board license number.
  5. Add client member ID / DOB only if needed for the OON claim.
  6. Enter DOS, CPT, ICD-10, units, fees, and payments.
  7. Telehealth: POS 10 or 02; modifier 95 when required.
  8. Keep sales tax aligned with Nebraska rules (see tax section)—usually $0 on therapy-only fees unless local advice says otherwise.
  9. Download the PDF locally—nothing is uploaded.

Two worked Nebraska examples

Nebraska's credential alphabet is unlike anywhere else on this site: the independent clinical license is LIMHP — Licensed Independent Mental Health Practitioner — while LMHP is the other tier, and neither resembles the LPC/LPCC pattern clinicians import from other states. The two documents below come from a fictional Omaha practice. All identifiers are placeholders.

Example 1 — an in-office private-pay Nebraska invoice

A LIMHP sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
Apr 3Individual psychotherapy, 45 min9083411$130.00$130.00
Apr 10Individual psychotherapy, 45 min9083411$130.00$130.00
Apr 17Individual psychotherapy, 45 min9083411$130.00$130.00
Apr 24Individual psychotherapy, 45 min9083411$130.00$130.00
Sales tax (see below)$0.00
Total$520.00$520.00

POS 11 is accurate for office sessions. The tax line stays at $0.00 because professional mental health services are generally exempt from Nebraska sales tax — therapy-only invoices carry no taxable line. The credential is LIMHP, Nebraska's independent mental health practice license, and the license field follows the NE LIMHP # hint — copy the number exactly as the DHHS license search displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Lincoln has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Jun 4907919510F33.1$160.00$160.00
Jun 11908349510F33.1$130.00$130.00
Jun 18908349510F33.1$130.00$130.00
Jun 25908349510F33.1$130.00$130.00
Total$550.00$550.00

The client was at home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.

The provider block carries the rendering name with LIMHP — the independent tier under Nebraska DHHS Licensure — plus the license number copied from the nebraska.gov LISSearch tool, NPI, practice address, and tax ID. If the rendering clinician were an independent clinical social worker, the credential would be LICSW. The paid column is populated because OON reimbursement repays actual expenditures.

Both parties were in Nebraska, so the state license sufficed. Nebraska has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.

Common mistakes on Nebraska superbills

  • Printing LPC or LPCC. Nebraska does not use those titles — the independent license is LIMHP. Out-of-state abbreviations fail to match DHHS records and can stall verification.
  • Confusing LMHP with LIMHP. LMHP is a different tier than the independent LIMHP. Most private-practice OON superbills should show LIMHP when that is the license held.
  • Printing LICSW loosely. Nebraska's independent clinical social work credential is LICSW — print the exact letters on the license, not the more common LCSW.
  • Treating enacted compact status as live. Nebraska is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Adding a sales-tax line. Professional mental health services are generally exempt from Nebraska sales tax. Leave the line at $0 unless you sell taxable goods.

FAQ

LMHP or LIMHP?

LIMHP is the independent practice license. Print LIMHP when you are independently licensed; print LMHP if that is the credential you currently hold.

Is Nebraska in the Counseling Compact?

Nebraska has enacted the compact but is not among the live/operational states as of this retrieval. Privileges work only when both home and remote states are live—check counselingcompact.gov.

Does Nebraska tax therapy sessions?

Professional mental health services are generally exempt from Nebraska sales tax. Leave tax at $0 on session-only invoices unless you sell taxable goods.

Can a Nebraska psychologist use PSYPACT?

Nebraska participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.

What do OON payers want on a Nebraska superbill?

Name + Nebraska credential (LIMHP or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.

Why these sources matter

Every factual claim on this page traces back to a primary source: Nebraska DHHS — Licensure Unit (Mental Health Practice), the official interstate-compact commissions, and Nebraska's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
