Missouri superbill and invoice for LPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Missouri selected. Out-of-network clients still need a superbill with your NPI, tax ID, Missouri license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Missouri superbills
Independent counseling in Missouri is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Missouri Committee for Professional Counselors. License line hint: MO LPC #.
Missouri uses LPC. PSYPACT is active for psychologists.
Telehealth coding (national rules, used in Missouri)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Missouri
Professional therapy services are generally exempt from Missouri sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Do Missouri superbills need a state seal?
No. A clean PDF with NPI, license, CPT, ICD-10, and dates of service is what OON payers process.
What to put on a Missouri superbill
- Your legal name, LPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Missouri license number as printed by the Missouri Committee for Professional Counselors
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Missouri clinicians use invoices for private-pay clients and superbills for out-of-network (OON) reimbursement. This generator produces either PDF entirely in the browser with Missouri defaults: LPC as the primary counseling title, a MO LPC # license hint, and standard NPI, CPT, ICD-10, place-of-service, and payment fields. No PHI is uploaded to a server.
Choose Missouri when your Missouri license (or another lawful pathway) and the client’s location for the session authorize care. Print the credential string on your Division of Professional Registration record. This page supports documentation only—it is not legal, billing, or tax advice.
License titles that belong on the form
Missouri independent counselors use Licensed Professional Counselor (LPC). That is the default for this generator. Clinical social workers commonly print LCSW; marriage and family therapists print LMFT; psychologists print Licensed Psychologist.
Exact credential dropdown strings:
`LPC` · `LCSW` · `LMFT` · `Licensed Psychologist`
Provisional or supervised counselor statuses are not interchangeable with independent LPC. If you practice under provisional authority, identify the supervisor in notes when a payer requires it and print only the credential you hold.
Social work and psychology are regulated through related Professional Registration pathways that are separate from the Committee for Professional Counselors—still use the exact letters on the license you are rendering under.
Board, license number, lookup
Professional counselors are regulated by the Missouri Committee for Professional Counselors within the Division of Professional Registration. Public verification is available through Missouri’s Licensee Search (primary-source verification updated from the licensing database).
Copy the license number exactly as Licensee Search displays it. The generator hint MO LPC # is a counselor cue; LCSW, LMFT, and psychologist licensees paste their own Missouri numbers in the same field.
Include Type 1 NPI (Type 2 for groups), tax ID, and practice address. Add taxonomy in notes when the plan requests it.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Counseling Compact — enacted (not yet operational for privileges in Missouri). Missouri appears on the Counseling Compact jurisdictions list. Enactment alone does not issue privileges. Live privilege states are listed separately on counselingcompact.gov; confirm Missouri’s operational status and the remote state’s live status before interstate practice.
PSYPACT — yes. Missouri participates for psychologists with required ASPPB authorizations. PSYPACT does not authorize LPC, LCSW, or LMFT practice.
Social Work Licensure Compact — enacted (not yet operational for multistate licenses). Missouri appears on the official Social Work Compact jurisdictions list. Multistate licenses are not yet available—verify at swcompact.org.
Telehealth + place of service
Use POS 10 when the patient is at home for telehealth, or POS 02 when telehealth occurs elsewhere. Add modifier 95 for synchronous telemedicine when the payer requires it. POS 11 is in-person office only.
Crossing state lines requires Missouri licensure, a live Counseling Compact privilege (when both states are live), PSYPACT for psychologists, or another lawful pathway. Print only credentials that authorize that date of service.
Sales tax on therapy invoices
Missouri sales and use tax primarily applies to retail sales of tangible personal property and to a limited set of enumerated services. Ordinary professional counseling and psychotherapy session fees are generally not among taxable services. Leave the invoice sales tax at $0 for clinical fees.
If you separately sell taxable goods, admissions, lodging-related items, or other listed taxable services, collect tax on those charges under Missouri Department of Revenue guidance. Confirm edge cases with DOR or a tax professional.

Documentation tips for Missouri OON claims
Missouri Division of Professional Registration Licensee Search is treated as primary-source verification. Before sending a client a superbill, confirm your public record shows active status and the same license number you printed. Name changes and address updates should be finished in the licensing system first so payer audits match the PDF.
If you hold multiple Missouri credentials (for example LPC and another behavioral health license), print only the credential under which the specific CPT service was rendered. Dual-credential clinicians sometimes confuse payer credentialing files by alternating titles without updating directories.
Because Missouri’s Counseling Compact status is enacted-but-verify-live, keep a dated screenshot or note of the Commission map when you rely on interstate authority, and avoid advertising multi-state practice until privileges actually appear in CompactConnect.
Two worked Missouri examples
Missouri keeps its counselor licensing inside the Division of Professional Registration under a committee rather than a board — the Committee for Professional Counselors — while social work and psychology live on sibling pathways, so the verification source follows the credential. The two documents below come from a fictional St. Louis practice. All identifiers are placeholders.
Example 1 — an in-office private-pay Missouri invoice
An LPC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Mar 11 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| Mar 18 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| Mar 25 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| Apr 1 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $500.00 | $500.00 | |||
POS 11 is correct for office sessions. The tax line stays at $0.00 because Missouri taxes tangible personal property and a limited set of enumerated services — ordinary professional therapy session fees are not among them. The credential is LPC, Missouri's independent counseling title, and the license field follows the MO LPC # hint — copy the number exactly as the state's Licensee Search displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Columbia has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Oct 7 | 90791 | 95 | 10 | F41.1 | $155.00 | $155.00 |
| Oct 14 | 90834 | 95 | 10 | F41.1 | $125.00 | $125.00 |
| Oct 21 | 90834 | 95 | 10 | F41.1 | $125.00 | $125.00 |
| Oct 28 | 90834 | 95 | 10 | F41.1 | $125.00 | $125.00 |
| Total | $530.00 | $530.00 | ||||
The client was at home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LPC, the license number copied from the Division of Professional Registration's Licensee Search, NPI, practice address, and tax ID. If the rendering clinician were a social worker or psychologist, the credential would come from those professions' own Missouri pathways — the committee's license is not theirs. The paid column is populated because OON reimbursement repays actual expenditures.
Both parties were in Missouri, so the state license sufficed. Missouri has enacted the Counseling Compact — privileges require both ends to be live on the Commission map — and participates in PSYPACT for psychologists holding ASPPB authorizations.
Common mistakes on Missouri superbills
- Printing LPC under a provisional license. Provisional and supervised counselor statuses are not interchangeable with independent LPC. Print the credential you hold and identify the supervisor in notes when the plan asks.
- Verifying the credential on the wrong pathway. Counselors verify through the Committee for Professional Counselors' Licensee Search; social workers and psychologists have separate Professional Registration pathways — use the one that issued the credential.
- Treating enacted compact status as live. Missouri is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Therapy session fees are outside Missouri's taxable-services base. Leave the line at $0 unless you sell taxable goods or listed services.
- Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments gives the adjuster nothing to act on.
FAQ
Do Missouri superbills need a state seal?
No. A clean PDF with NPI, license, CPT, ICD-10, dates of service, charges, and amount paid is what OON payers process.
Is Missouri Counseling Compact operational?
Missouri has enacted the compact. Privileges require live status on the Commission map—verify before interstate care.
Does Missouri tax therapy?
Ordinary professional therapy fees are generally not sales-taxable. Leave tax at $0 unless you sell taxable goods or enumerated services.
What do OON payers want?
Provider name, LPC/LCSW/LMFT/psychologist, Missouri license number, NPI, address, DOS, CPT, ICD-10, POS, charges, and amount paid.
Provisional counselors?
Use the provisional credential you hold and identify the supervisor when required—do not list independent LPC early.
Why these sources matter
Every factual claim on this page traces back to a primary source: Missouri Committee for Professional Counselors, the official interstate-compact commissions, and Missouri's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
