District of Columbia superbill and invoice for LPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with District of Columbia selected. Out-of-network clients still need a superbill with your NPI, tax ID, District of Columbia license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on District of Columbia superbills
Independent counseling in District of Columbia is commonly written as LPC. Also print the letters you actually hold: LPC, LICSW, LMFT, Licensed Psychologist. The board to match your number against is the DC Board of Professional Counseling. License line hint: DC LPC #.
DC social workers use LICSW rather than LCSW. PSYPACT includes the District.
Telehealth coding (national rules, used in District of Columbia)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in District of Columbia
Professional mental health services are generally not subject to DC sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Not a member
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Do I put Washington, DC or District of Columbia on the form?
Use “Washington, DC” plus ZIP 200xx in the address block. For state/jurisdiction on the license line, “DC” is enough.
What to put on a District of Columbia superbill
- Your legal name, LPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- District of Columbia license number as printed by the DC Board of Professional Counseling
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
District of Columbia private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, District of Columbia license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select District of Columbia defaults when your rendering authority and the client's location for the session are District of Columbia (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
The District's independent counseling title is Licensed Professional Counselor (LPC). Supervised counselors often hold LGPC—print that string while under supervision. Independent clinical social workers commonly use LICSW rather than LCSW.
Other titles commonly used on District of Columbia superbills:
- LGPC — print the exact letters on your District of Columbia license/board record.
- LICSW — print the exact letters on your District of Columbia license/board record.
- LMFT — print the exact letters on your District of Columbia license/board record.
- Licensed Psychologist — print the exact letters on your District of Columbia license/board record.
Exact credential dropdown strings:
`LPC` · `LGPC` · `LICSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Counselor licenses are issued by the DC Board of Professional Counseling through DC Health. Verify via the District professional license lookup. On address lines use Washington, DC and a 200xx ZIP; on the jurisdiction field DC is enough.
Official verification: https://doh.dc.gov/service/professional-license-lookup. Copy the license number exactly as shown. The generator hint DC LPC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — enacted (not yet operational for privileges). District of Columbia appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
- PSYPACT — yes. District of Columbia participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
- Social Work Licensure Compact — no at retrieval (District of Columbia not listed on swcompact.org/compact-jurisdictions). Do not advertise multistate SW practice via the compact until the official map includes District of Columbia.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A District of Columbia license, a live Counseling Compact privilege naming District of Columbia (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
DC sales and use tax generally covers tangible personal property and certain taxable services. Professional mental health counseling/psychotherapy is generally not subject to DC sales tax. Leave tax at $0 on clinical invoices. Confirm with OTR or a DC preparer if you also sell goods.

Two worked District of Columbia examples
DC paperwork has quirks all its own: the licensing agency is DC Health rather than a professional-regulation department, the clinical social work credential is LICSW, and the address block should read "Washington, DC" even though the jurisdiction field just needs "DC". The two documents below come from a fictional Dupont Circle practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay District of Columbia invoice
An LPC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Apr 3 | Individual psychotherapy, 45 min | 90834 | 11 | $180.00 | $180.00 |
| Apr 10 | Individual psychotherapy, 45 min | 90834 | 11 | $180.00 | $180.00 |
| Apr 17 | Individual psychotherapy, 45 min | 90834 | 11 | $180.00 | $180.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $540.00 | $540.00 | |||
POS 11 is accurate for office sessions. The tax line reads $0.00 because the District's sales and use tax covers tangible personal property and certain taxable services, and professional mental health counseling is generally not subject to it — confirm with the Office of Tax and Revenue if you also sell goods. The credential is LPC, and the license field follows the DC LPC # hint — copy the number exactly as the DC Health professional license lookup displays it. The address block reads "Washington, DC" with a 200xx ZIP; the jurisdiction field is simply DC.
Example 2 — a telehealth out-of-network superbill
A second client in Capitol Hill has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Jul 9 | 90791 | 95 | 10 | F41.1 | $215.00 | $215.00 |
| Jul 16 | 90834 | 95 | 10 | F41.1 | $180.00 | $180.00 |
| Jul 23 | 90834 | 95 | 10 | F41.1 | $180.00 | $180.00 |
| Jul 30 | 90834 | 95 | 10 | F41.1 | $180.00 | $180.00 |
| Total | $755.00 | $755.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block shows the rendering name with LPC, the DC Health license number copied from the official lookup, NPI, the Washington, DC practice address, and tax ID. If the rendering clinician were a clinical social worker, the credential would be LICSW — the District's independent clinical title, not plain LCSW. The paid column is populated because OON reimbursement repays money already spent.
Both parties were in the District, so the DC Health license sufficed. The District of Columbia has enacted the Counseling Compact but was not among the live, operational jurisdictions at retrieval — privileges require both ends to be live. DC does participate in PSYPACT for psychologists, and it is not on the Social Work Compact jurisdictions list — one of the few compact maps where the District genuinely differs from its neighbors.
Common mistakes on District of Columbia superbills
- Printing LCSW instead of LICSW. The District's independent clinical social work credential is LICSW. Match the exact letters on the DC Health record — the neighboring states' conventions do not apply.
- Printing LPC while holding LGPC. Licensed Graduate Professional Counselors practice under supervision. Print LGPC and identify the supervisor when the plan asks.
- Treating enacted compact status as live. The District has enacted the Counseling Compact but is not among the operational jurisdictions — verify counselingcompact.gov before relying on a privilege.
- Assuming Social Work Compact membership. DC is not on the swcompact.org jurisdictions list. Do not advertise multistate social work practice on the strength of a DC license.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Professional counseling is generally not subject to DC sales tax. Leave the line at $0 unless you sell taxable goods — and confirm with OTR if you do.

Why these sources matter
Every factual claim on this page traces back to a primary source: DC Board of Professional Counseling (DC Health), the official interstate-compact commissions, and District of Columbia's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
