Utah superbill and invoice for LCMHC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Utah selected. Out-of-network clients still need a superbill with your NPI, tax ID, Utah license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Utah superbills
Independent counseling in Utah is commonly written as LCMHC. Also print the letters you actually hold: LCMHC, LCSW, LMFT, Licensed Psychologist, ACMHC. The board to match your number against is the Utah Division of Professional Licensing — Mental Health. License line hint: UT LCMHC #.
Utah counselors are LCMHCs (associates: ACMHC).
Telehealth coding (national rules, used in Utah)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Utah
Professional mental health services are generally exempt from Utah sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Can an ACMHC issue the superbill?
Disclose the associate title and supervisor. Many commercial plans still process OON superbills if the rendering line is accurate.
What to put on a Utah superbill
- Your legal name, LCMHC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Utah license number as printed by the Utah Division of Professional Licensing — Mental Health
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Utah private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Utah license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select Utah defaults when your rendering authority and the client’s location for the session are Utah (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your Utah Division of Professional Licensing (DOPL) record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
Utah’s independent clinical counseling title is Licensed Clinical Mental Health Counselor (LCMHC). That is the generator default—not “LPC.” Pre-independent counselors use Associate Clinical Mental Health Counselor (ACMHC) while accruing hours—print ACMHC, not LCMHC, during the associate period.
Other titles commonly used on Utah superbills:
- LCSW — Licensed Clinical Social Worker.
- LMFT — Licensed Marriage and Family Therapist.
- Licensed Psychologist — psychologists licensed through DOPL / psychology pathways—print the exact title on your license.
- ACMHC — Associate Clinical Mental Health Counselor (supervised).
Exact credential dropdown strings:
`LCMHC` · `LCSW` · `LMFT` · `Licensed Psychologist` · `ACMHC`
Do not put “LCMHC” on the form if you only hold ACMHC. Associates should identify the supervisor when a payer asks. DOPL has published guidance on supervisor training requirements effective January 1, 2027—confirm current rules on the Clinical Mental Health Counseling page.
Board, license number, lookup
Clinical mental health counselor, social work, MFT, and related licenses are administered by the Utah Division of Professional Licensing (DOPL) under the Department of Commerce — Clinical Mental Health Counseling.
Official verification: Utah Licensee Lookup & Verification System. Copy the license number exactly as shown. The generator hint UT LCMHC # is a placeholder—replace it with your real DOPL number. Include individual NPI (and group NPI if applicable) plus tax ID as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — enacted (not yet operational for privileges). Utah appears on counselingcompact.gov/map/compact-states. DOPL links to compact information on the CMHC page. Privileges are usable only when both home and remote states are live/operational.
- PSYPACT — yes. Utah participates for psychologists with required ASPPB authorizations. Confirm on psypact.gov.
- Social Work Licensure Compact — enacted. Utah is listed on swcompact.org/compact-jurisdictions. Multistate SW licenses are not yet issuing—confirm at swcompact.org.
Verify before you rely on it. Compact membership and privilege systems change.
Telehealth + place of service
Use POS 10 when the patient is at home, or POS 02 for telehealth elsewhere. Add modifier 95 for synchronous telemedicine when the payer’s guide requires it. Do not use POS 11 for a video-only visit.
A Utah LCMHC license, a live Counseling Compact privilege naming Utah (when operational), or another lawful authorization must cover the client’s location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Utah sales and use tax applies to retail sales of tangible personal property, products transferred electronically, and certain services enumerated in Utah Code §59-12-103 (see Utah State Tax Commission Publication 25). Professional consulting and clinical mental health counseling session fees are generally not among those enumerated taxable services. Tax Commission advisory opinions treat pure professional consulting as nontaxable when no taxable goods are sold.
Leave the invoice sales tax at $0 for clinical services. If you also sell taxable books, materials, or other tangible property, collect tax on those goods. Confirm edge cases with the Utah State Tax Commission or a tax professional.

Two worked Utah examples
Utah's paperwork runs through DOPL — the Division of Professional Licensing — and its credential trap is direction-specific: LCMHC is the independent title, ACMHC the associate tier, and "LPC" does not appear on Utah licenses at all. The two documents below come from a fictional Salt Lake City practice. All identifiers are placeholders.
Example 1 — an in-office private-pay Utah invoice
An LCMHC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Mar 4 | Individual psychotherapy, 45 min | 90834 | 11 | $135.00 | $135.00 |
| Mar 11 | Individual psychotherapy, 45 min | 90834 | 11 | $135.00 | $135.00 |
| Mar 18 | Individual psychotherapy, 45 min | 90834 | 11 | $135.00 | $135.00 |
| Mar 25 | Individual psychotherapy, 45 min | 90834 | 11 | $135.00 | $135.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $540.00 | $540.00 | |||
POS 11 is accurate for office sessions. The tax line stays at $0.00 because Utah taxes tangible personal property, electronically transferred products, and certain enumerated services under Utah Code §59-12-103 — professional mental health counseling is not among the taxable services. The credential is LCMHC, and the license field follows the UT LCMHC # hint — copy the number exactly as the Licensee Lookup & Verification System displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Provo has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Jun 9 | 90791 | 95 | 10 | F41.1 | $160.00 | $160.00 |
| Jun 16 | 90834 | 95 | 10 | F41.1 | $135.00 | $135.00 |
| Jun 23 | 90834 | 95 | 10 | F41.1 | $135.00 | $135.00 |
| Jun 30 | 90834 | 95 | 10 | F41.1 | $135.00 | $135.00 |
| Total | $565.00 | $565.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LCMHC, the license number copied verbatim from DOPL's secure.utah.gov lookup, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures — a blank paid column gives the plan nothing to reimburse.
Both parties were in Utah, so the state license sufficed. Utah has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations. An ACMHC delivering these sessions would print the associate credential and identify the supervisor when the plan asks.
Common mistakes on Utah superbills
- Writing "LPC" instead of "LCMHC". Utah's independent clinical counseling title is Licensed Clinical Mental Health Counselor. LPC strings fail to match DOPL records and can stall payer verification.
- Printing LCMHC while holding ACMHC. The Associate Clinical Mental Health Counselor credential is the supervised tier — print ACMHC and identify the supervisor when the plan asks.
- Ignoring evolving supervision rules. DOPL has published guidance on supervisor training requirements effective January 1, 2027 — associates and supervisors should confirm current rules on the Clinical Mental Health Counseling page.
- Treating enacted compact status as live. Utah is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Counseling is not among Utah's enumerated taxable services under §59-12-103. Leave the line at $0 unless you sell taxable goods.
FAQ
Can an ACMHC issue the superbill?
Disclose the associate title and supervisor. Many commercial plans still process OON superbills if the rendering line is accurate—confirm with the plan first.
Is Utah in the Counseling Compact?
Yes—enacted. Privileges work only with other live/operational states; check counselingcompact.gov and DOPL compact notes before interstate counseling claims.
Does Utah tax therapy sessions?
Professional mental health services are generally exempt from Utah sales tax (Pub 25 / §59-12-103 pattern). Leave tax at $0 on session-only invoices unless you sell taxable goods.
Can a Utah psychologist use PSYPACT?
Utah participates in PSYPACT. You still need active authorizations—verify on psypact.gov.
What do OON payers want on a Utah superbill?
Name + Utah credential (LCMHC or ACMHC as held), DOPL license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Utah Division of Professional Licensing (DOPL) — Mental Health / Clinical Mental Health Counseling, the official interstate-compact commissions, and Utah's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
