Delaware superbill and invoice for LPCMH, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Delaware selected. Out-of-network clients still need a superbill with your NPI, tax ID, Delaware license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Delaware superbills

Independent counseling in Delaware is commonly written as LPCMH. Also print the letters you actually hold: LPCMH, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Delaware Board of Mental Health and Chemical Dependency Professionals. License line hint: DE LPCMH #.

Delaware's independent counseling title is Licensed Professional Counselor of Mental Health (LPCMH), not LPC.

Telehealth coding (national rules, used in Delaware)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Delaware

Delaware has no state sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

Why doesn't Delaware use LPC?

The statute uses LPCMH / LACMH. Put “LPCMH” after your name so payers do not treat the credential as out of state.

What to put on a Delaware superbill

  • Your legal name, LPCMH or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Delaware license number as printed by the Delaware Board of Mental Health and Chemical Dependency Professionals
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Delaware private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Delaware license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select Delaware defaults when your rendering authority and the client's location for the session are Delaware (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

Delaware's independent counseling title is Licensed Professional Counselor of Mental Health (LPCMH)—not LPC. Associates use Licensed Associate Counselor of Mental Health (LACMH). Using the wrong letters confuses commercial payers.

Other titles commonly used on Delaware superbills:

  • LACMH — print the exact letters on your Delaware license/board record.
  • LCSW — print the exact letters on your Delaware license/board record.
  • LMFT — print the exact letters on your Delaware license/board record.
  • Licensed Psychologist — print the exact letters on your Delaware license/board record.

Exact credential dropdown strings:

`LPCMH` · `LACMH` · `LCSW` · `LMFT` · `Licensed Psychologist`

Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.

Board, license number, lookup

Licenses are issued by the Board of Mental Health and Chemical Dependency Professionals under the Division of Professional Regulation. Verify free via DELPROS Search & Verify License (primary-source for credentialing).

Official verification: https://delpros.delaware.gov/OH_VerifyLicense. Copy the license number exactly as shown. The generator hint DE LPCMH # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — enacted (not yet operational for privileges). Delaware appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
  • PSYPACT — yes. Delaware participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
  • Social Work Licensure Compact — enacted. Delaware is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.

Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.

Telehealth + place of service

For video psychotherapy superbills, use the place-of-service codes payers expect:

  • POS 10 — patient at home
  • POS 02 — telehealth other than home
  • Modifier 95 — when the plan wants synchronous telemedicine marked

Do not code a video-only session as POS 11. A Delaware license, a live Counseling Compact privilege naming Delaware (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

Delaware has no state sales tax. Do not add a client-facing sales-tax line on therapy invoices. Any gross receipts or business tax obligations are separate from the invoice tax field—handle those with your CPA. See the Delaware Division of Revenue.

How to fill the Delaware generator

  1. Choose Superbill or Invoice.
  2. Set jurisdiction to DE.
  3. Enter rendering name; select LPCMH (default) or another credential you currently hold from the dropdown.
  4. Complete practice demographics, NPI, tax ID, and Delaware board license number.
  5. Add client member ID / DOB only if needed for the OON claim.
  6. Enter DOS, CPT, ICD-10, units, fees, and payments.
  7. Telehealth: POS 10 or 02; modifier 95 when required.
  8. Keep sales tax aligned with Delaware rules (see tax section)—usually $0 on therapy-only fees unless local advice says otherwise.
  9. Download the PDF locally—nothing is uploaded.

Two worked Delaware examples

Delaware paperwork diverges from its neighbors in two ways that matter on every document: the counseling credential is LPCMH — Licensed Professional Counselor of Mental Health, not LPC — and Delaware has no state sales tax at all. The two documents below come from a fictional Wilmington practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.

Example 1 — an in-office private-pay Delaware invoice

An LPCMH sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
May 8Individual psychotherapy, 60 min9083711$160.00$160.00
May 15Individual psychotherapy, 60 min9083711$160.00$160.00
May 22Individual psychotherapy, 60 min9083711$160.00$160.00
May 29Individual psychotherapy, 60 min9083711$160.00$160.00
Sales tax (see below)$0.00
Total$640.00$640.00

POS 11 is correct for office sessions. The tax line stays at $0.00 — but not because counseling is exempt: Delaware simply has no state sales tax. Gross receipts and business taxes are a separate obligation handled with your CPA, never a client-facing invoice line. The credential is LPCMH, and the license field follows the DE LPCMH # hint — copy the number exactly as the DELPROS Search & Verify tool displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Newark has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth place of service, the telemedicine modifier, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Oct 6907919510F33.1$190.00$190.00
Oct 13908379510F33.1$160.00$160.00
Oct 20908379510F33.1$160.00$160.00
Oct 27908379510F33.1$160.00$160.00
Total$670.00$670.00

The client was at home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous audio-video service for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.

The provider block carries the rendering name with LPCMH — printing "LPC" invites a payer to treat the credential as out-of-state — plus the license number copied from DELPROS, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures.

Both parties were in Delaware, so the state license sufficed. Delaware has enacted the Counseling Compact but was not among the live operational jurisdictions at retrieval, and it has enacted the Social Work Compact though multistate licenses are not yet issuing. The District-sized caution applies: enacted is not operational — verify both ends on the official maps before any interstate reliance.

Common mistakes on Delaware superbills

  • Writing "LPC" instead of "LPCMH". Delaware's statute uses Licensed Professional Counselor of Mental Health. An LPC string reads as an out-of-state credential to payer processors — print LPCMH.
  • Printing LPCMH while holding LACMH. Licensed Associate Counselors of Mental Health practice under supervision. Print LACMH and identify the supervisor when the plan asks.
  • Adding a sales-tax line out of habit. Delaware has no state sales tax at all. Any gross-receipts obligation is a business tax handled off-invoice with your CPA — not a client-facing line.
  • Treating enacted compact status as live. Delaware's Counseling Compact membership is enacted but not operational — verify counselingcompact.gov before relying on a privilege.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments gives the adjuster nothing to act on.

FAQ

Why doesn't Delaware use LPC?

The statute uses LPCMH / LACMH. Put LPCMH after your name so payers do not treat the credential as out of state. Associates must print LACMH.

Is Delaware in the Counseling Compact?

Delaware has enacted the compact but is not among the live/operational states as of this retrieval. Privileges work only when both home and remote states are live—check counselingcompact.gov.

Any tax line on a Delaware invoice?

No statewide sales tax. Leave tax at $0 unless you have a specific non-sales-tax obligation your preparer instructs you to show.

Can a Delaware psychologist use PSYPACT?

Delaware participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.

What do OON payers want on a Delaware superbill?

Name + Delaware credential (LPCMH or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.

Why these sources matter

Every factual claim on this page traces back to a primary source: Delaware Board of Mental Health and Chemical Dependency Professionals, the official interstate-compact commissions, and Delaware's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
