Louisiana superbill and invoice for LPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Louisiana selected. Out-of-network clients still need a superbill with your NPI, tax ID, Louisiana license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Louisiana superbills
Independent counseling in Louisiana is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Louisiana LPC Board / LABSWE. License line hint: LA LPC #.
Louisiana LPCs are compact-operational in the early wave. Psychologists: Louisiana is not a PSYPACT participant.
Telehealth coding (national rules, used in Louisiana)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Louisiana
Professional mental health services are generally not subject to Louisiana sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Operational / issuing privileges
- PSYPACT (psychologists): Not a member
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Can a Louisiana psychologist use PSYPACT?
No. Louisiana is outside PSYPACT. Interstate telepsychology requires another legal pathway, usually a Louisiana license.
What to put on a Louisiana superbill
- Your legal name, LPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Louisiana license number as printed by the Louisiana LPC Board / LABSWE
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Louisiana private-practice clinicians use superbills for out-of-network (OON) reimbursement and invoices for cash-pay clients. This generator produces either document entirely in the browser: no account, no PHI upload, and a downloadable PDF with rendering credential, Louisiana license string, NPI, CPT, ICD-10, place of service, and amounts.
Choose Louisiana defaults when the rendering provider’s authority and the client’s location for the session are Louisiana (or another state you are lawfully authorized to serve, including via a live Counseling Compact privilege). Match the exact title on your Louisiana LPC Board or LABSWE record. This page supports documentation workflow only; it is not legal, billing, or tax advice.
License titles that belong on the form
Louisiana’s independent counseling title is Licensed Professional Counselor (LPC). That is the generator default. Pre-independent counselors use Provisional Licensed Professional Counselor (PLPC) under board supervision rules—print PLPC, not LPC, while accruing hours.
Other titles commonly used on Louisiana superbills:
- LCSW — Licensed Clinical Social Worker (Louisiana State Board of Social Work Examiners / LABSWE).
- LMFT — Licensed Marriage and Family Therapist (print the exact letters on your license).
- Licensed Psychologist — psychologists licensed by the Louisiana State Board of Examiners of Psychologists (LSBEP)—print the exact title on your license.
Exact credential dropdown strings:
`LPC` · `LCSW` · `LMFT` · `Licensed Psychologist`
Do not put “LPC” on the form if you only hold PLPC. Identify the supervisor on associate/provisional lines when a payer asks.
Board, license number, lookup
LPC and PLPC licenses are issued by the Louisiana Licensed Professional Counselors Board of Examiners — lpcboard.org. Social workers verify through LABSWE; psychologists through LSBEP.
Official counselor verification: use the Board’s public licensee search and formal verification of licensure process when a payer needs a sealed letter. Copy the license number exactly as shown. The generator hint LA LPC # is a placeholder—replace it with your real Board number. Include individual NPI (and group NPI if applicable) plus tax ID as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — operational. Louisiana is among the early live privilege states on the Counseling Compact. Louisiana-licensed LPCs who meet eligibility may apply for privileges in other live compact states; counselors from other live states may obtain privileges to practice in Louisiana. Associates/PLPCs are not eligible for privileges. Confirm both home and remote states on counselingcompact.gov.
- PSYPACT — no (not yet effective). Louisiana enacted PSYPACT legislation (Act 302 / HB 486 of the 2026 Regular Session), but LSBEP states participation becomes effective January 1, 2028. Until that date, Louisiana psychologists and out-of-state psychologists cannot use PSYPACT authority for Louisiana practice. See lsbep.org/psypact.
- Social Work Licensure Compact — enacted. Louisiana is listed on swcompact.org/compact-jurisdictions. Multistate SW licenses are not yet issuing—confirm at swcompact.org.
Verify before you rely on it. Compact membership and go-live dates change.
Telehealth + place of service
Use POS 10 when the patient is at home, or POS 02 for telehealth elsewhere. Add modifier 95 for synchronous telemedicine when the payer’s guide requires it. Do not use POS 11 for a video-only visit.
A Louisiana LPC license, a live Counseling Compact privilege naming Louisiana, or another lawful authorization must cover the client’s location. Psychologists cannot rely on PSYPACT into or out of Louisiana until the 2028 effective date.
Sales tax on therapy invoices
Louisiana state sales tax applies only to enumerated service categories identified in law, including La. R.S. 47:301.3. The Louisiana Department of Revenue lists ten taxable service categories (sleeping rooms, admissions, parking, printing/copying, laundry/cleaning, cold storage, repairs of tangible personal property and digital products, telecommunications, prewritten computer software access, and information services). Professional psychotherapy and counseling session fees are not among those categories.
Leave the invoice sales tax at $0 for clinical services. Confirm edge cases (bundled taxable goods, local rules) with the Louisiana Department of Revenue or a Louisiana tax professional.

Two worked Louisiana examples
Louisiana paperwork has two signatures: the supervised counseling tier is PLPC — Provisional Licensed Professional Counselor — and Louisiana is one of the few states where the Counseling Compact is genuinely operational. The two documents below come from a fictional Baton Rouge practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Louisiana invoice
An LPC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| May 5 | Individual psychotherapy, 45 min | 90834 | 11 | $130.00 | $130.00 |
| May 12 | Individual psychotherapy, 45 min | 90834 | 11 | $130.00 | $130.00 |
| May 19 | Individual psychotherapy, 45 min | 90834 | 11 | $130.00 | $130.00 |
| May 26 | Individual psychotherapy, 45 min | 90834 | 11 | $130.00 | $130.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $520.00 | $520.00 | |||
POS 11 is accurate for office sessions. The tax line stays at $0.00 because Louisiana's state sales tax applies only to enumerated service categories in La. R.S. 47:301.3 — lodging, admissions, parking, telecom, certain repairs — and professional counseling is not among them. The credential is LPC, and the license field follows the LA LPC # hint — copy the number exactly as the LPC Board's licensee search displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Lafayette has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Jul 8 | 90791 | 95 | 10 | F41.0 | $160.00 | $160.00 |
| Jul 15 | 90834 | 95 | 10 | F41.0 | $130.00 | $130.00 |
| Jul 22 | 90834 | 95 | 10 | F41.0 | $130.00 | $130.00 |
| Jul 29 | 90834 | 95 | 10 | F41.0 | $130.00 | $130.00 |
| Total | $550.00 | $550.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LPC, the license number copied from lpcboard.org's licensee search — the board also offers formal sealed verification letters when a payer demands one — plus NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures.
Both parties were in Louisiana, so the state license sufficed. Louisiana is among the operational Counseling Compact states — privileges are actually issuing — so a Louisiana LPC can hold privileges in other live compact states. Two cautions ride along: the privilege must still name the remote state, and Louisiana does not participate in PSYPACT, so the psychologist in the group next door has no parallel pathway from this state.
Common mistakes on Louisiana superbills
- Printing LPC while holding PLPC. Provisional Licensed Professional Counselors practice under board supervision while accruing hours. Print PLPC and identify the supervisor when the plan asks.
- Assuming compact privileges cover provisional licensees. Louisiana's operational Counseling Compact extends to independently licensed counselors — PLPC is not a privilege credential.
- Assuming PSYPACT covers Louisiana. Louisiana does not participate in PSYPACT — psychologists need Louisiana licensure or another lawful pathway for clients located here.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Counseling is not among Louisiana's enumerated taxable services under La. R.S. 47:301.3. Leave the line at $0 unless you sell taxable goods.
- Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments gives the adjuster nothing to act on.
FAQ
Can a Louisiana psychologist use PSYPACT?
No—not until January 1, 2028 per LSBEP. Interstate telepsychology into or from Louisiana requires another lawful pathway (typically a Louisiana license) until PSYPACT becomes effective.
Is Louisiana in the Counseling Compact?
Yes—operational. Privileges work only with other live states; verify counselingcompact.gov before applying or practicing.
Can a PLPC issue an OON superbill?
Disclose the provisional/associate title and supervisor. Many commercial plans still process OON superbills if the rendering line is accurate—confirm with the plan first.
Does Louisiana tax therapy sessions?
Psychotherapy is not among the enumerated taxable service categories in La. R.S. 47:301.3. Leave tax at $0 on session-only invoices unless you sell taxable items or listed services.
What do OON payers want on a Louisiana superbill?
Name + Louisiana credential, board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Louisiana Licensed Professional Counselors Board of Examiners (LPC Board); Louisiana State Board of Social Work Examiners (LABSWE), the official interstate-compact commissions, and Louisiana's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
