Alaska superbill and invoice for LPC, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Alaska selected. Out-of-network clients still need a superbill with your NPI, tax ID, Alaska license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Alaska superbills

Independent counseling in Alaska is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Alaska Board of Professional Counselors. License line hint: AK LPC #.

Alaska is outside PSYPACT and the Counseling Compact. Telehealth into Alaska generally requires an Alaska license.

Telehealth coding (national rules, used in Alaska)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Alaska

Alaska has no state sales tax. A few municipalities levy local sales tax; professional therapy is typically not taxed. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Not a member
  • PSYPACT (psychologists): Pending / verify current status
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

Does Alaska tax therapy sessions?

There is no statewide sales tax. Check the borough or city if you have a physical office in a locality that taxes selected services.

What to put on a Alaska superbill

  • Your legal name, LPC or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Alaska license number as printed by the Alaska Board of Professional Counselors
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Alaska private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Alaska license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select Alaska defaults when your rendering authority and the client's location for the session are Alaska (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

Alaska's independent counseling title is Licensed Professional Counselor (LPC). That is the generator default. An Associate Counselor pathway has been authorized in statute; board materials have noted implementing regulations may still be completing—print only the credential currently shown on your CBPL record.

Other titles commonly used on Alaska superbills:

  • LCSW — print the exact letters on your Alaska license/board record.
  • LMFT — print the exact letters on your Alaska license/board record.
  • Licensed Psychologist — print the exact letters on your Alaska license/board record.

Exact credential dropdown strings:

`LPC` · `LCSW` · `LMFT` · `Licensed Psychologist`

Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.

Board, license number, lookup

Counselor licenses are issued by the Alaska Board of Professional Counselors, staffed by CBPL. Social work and psychology licenses verify through their respective Alaska boards on CBPL search tools.

Official verification: https://www.commerce.alaska.gov/cbp/main/Search/Professional. Copy the license number exactly as shown. The generator hint AK LPC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — no. Alaska is not listed as a member jurisdiction on counselingcompact.gov/map/compact-states. Do not rely on compact privileges to counsel clients located in Alaska.
  • PSYPACT — pending. Alaska enacted PSYPACT legislation in 2026 (psypact.gov legislative updates). Treat participation as pending until Alaska appears as active on the official PSYPACT map and you hold ASPPB authorizations—verify before interstate psychology work.
  • Social Work Licensure Compact — enacted. Alaska enacted the compact (HB 110; reported on swcompact.org news July 2026). Multistate SW licenses are not yet issuing.

Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.

Telehealth + place of service

For video psychotherapy superbills, use the place-of-service codes payers expect:

  • POS 10 — patient at home
  • POS 02 — telehealth other than home
  • Modifier 95 — when the plan wants synchronous telemedicine marked

Do not code a video-only session as POS 11. A Alaska license, a live Counseling Compact privilege naming Alaska (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

Alaska has no statewide sales tax. Some boroughs and cities levy local sales taxes. Professional counseling and psychotherapy session fees are typically outside a retail sales-tax pattern. Leave the invoice sales tax at $0 for clinical services unless you have a documented local collection duty or sell taxable goods. Confirm with the Alaska Department of Revenue or a local preparer.

How to fill the Alaska generator

  1. Choose Superbill or Invoice.
  2. Set jurisdiction to AK.
  3. Enter rendering name; select LPC (default) or another credential you currently hold from the dropdown.
  4. Complete practice demographics, NPI, tax ID, and Alaska board license number.
  5. Add client member ID / DOB only if needed for the OON claim.
  6. Enter DOS, CPT, ICD-10, units, fees, and payments.
  7. Telehealth: POS 10 or 02; modifier 95 when required.
  8. Keep sales tax aligned with Alaska rules (see tax section)—usually $0 on therapy-only fees unless local advice says otherwise.
  9. Download the PDF locally—nothing is uploaded.

Two worked Alaska examples

Alaska's billing paperwork is lighter than most jurisdictions in one respect — there is no statewide sales tax — but heavier in another, because the state sits outside the Counseling Compact entirely. The two documents below come from a fictional Anchorage practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.

Example 1 — an in-office private-pay Alaska invoice

An LPC sees a self-pay client for three weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — a record of services, charges, and payments — and the diagnosis column can stay empty since no claim will be filed against it.

Date of serviceServiceCPTPOSChargePaid
Sep 9Individual psychotherapy, 60 min9083711$185.00$185.00
Sep 16Individual psychotherapy, 60 min9083711$185.00$185.00
Sep 23Individual psychotherapy, 60 min9083711$185.00$185.00
Sales tax (see below)$0.00
Total$555.00$555.00

POS 11 is accurate — every session was in the office. The tax line reads $0.00, but for a different reason than most states: Alaska levies no statewide sales tax at all. Some boroughs and cities do tax selected services, so the $0 here reflects that this practice has no documented local collection duty — confirm your municipality's rules if you have a physical office somewhere that taxes services. The credential is LPC, and the license field follows the AK LPC # hint — copy the number exactly as the CBPL professional search displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Juneau has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth place of service, the telemedicine modifier, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Oct 2907919510F41.0$210.00$210.00
Oct 9908379510F41.0$185.00$185.00
Oct 16908379510F41.0$185.00$185.00
Oct 23908379510F41.0$185.00$185.00
Total$765.00$765.00

The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous audio-video service for plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.

The provider block carries the rendering name with LPC, the CBPL license number copied verbatim from the commerce.alaska.gov verification search, NPI, practice address, and tax ID. The paid column is populated because out-of-network reimbursement repays actual expenditures — a blank paid column gives the plan nothing to reimburse.

Both parties were in Alaska throughout, which mattered more here than in compact states: Alaska is not a Counseling Compact member, so no privilege pathway exists for an out-of-state counselor reaching an Alaska client — a Alaska license or another lawful authorization is required. Alaska did enact PSYPACT legislation in 2026 and the Social Work Compact via HB 110, but PSYPACT participation remains pending until the official map shows it active, and multistate social work licenses are not yet issuing.

Common mistakes on Alaska superbills

  • Assuming a compact covers Alaska. Alaska is outside the Counseling Compact entirely. A home-state counseling license from another state does not authorize treating a client located in Alaska.
  • Treating PSYPACT as live. Alaska enacted PSYPACT legislation in 2026, but participation is pending until the official map shows Alaska active and you hold ASPPB authorizations — verify before any interstate psychology work.
  • Printing an associate title as LPC. Alaska's Associate Counselor pathway is authorized in statute with implementing regulations completing — print only the credential shown on your CBPL record, and identify the supervisor when a payer asks.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an in-person office visit.
  • Assuming $0 tax needs no thought. Alaska has no statewide sales tax, but municipalities can levy their own — confirm local obligations before treating the tax line as automatic.
  • Leaving the paid column blank. OON plans reimburse money already spent. A superbill showing charges without payments usually comes back unanswered.

FAQ

Can a pre-licensed Alaska counselor issue a superbill?

Many commercial payers expect an independently licensed rendering provider. Print only the title you currently hold and identify the supervisor when asked. Confirm with the client's plan before an associate-level OON claim.

Is Alaska in the Counseling Compact?

No—Alaska is outside the Counseling Compact. Telehealth or in-person practice into Alaska generally requires a Alaska license (or another lawful authorization).

Does Alaska tax therapy sessions?

There is no statewide sales tax. Check the borough or city if you have a physical office in a locality that taxes selected services.

Can a Alaska psychologist use PSYPACT?

Alaska has enacted PSYPACT but is treated as pending until the official map shows it as active. Verify on psypact.gov before advertising interstate psychology.

What do OON payers want on a Alaska superbill?

Name + Alaska credential (LPC or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.

Why these sources matter

Every factual claim on this page traces back to a primary source: Alaska Board of Professional Counselors (CBPL), the official interstate-compact commissions, and Alaska's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
