Oklahoma superbill and invoice for LPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Oklahoma selected. Out-of-network clients still need a superbill with your NPI, tax ID, Oklahoma license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Oklahoma superbills
Independent counseling in Oklahoma is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Oklahoma State Board of Behavioral Health Licensure. License line hint: OK LPC #.
Oklahoma LPCs are licensed by the BBHL. PSYPACT is active.
Telehealth coding (national rules, used in Oklahoma)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Oklahoma
Professional therapy is generally exempt from Oklahoma sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Associate titles in Oklahoma?
If you hold a candidate/under-supervision credential, print that title rather than LPC.
What to put on a Oklahoma superbill
- Your legal name, LPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Oklahoma license number as printed by the Oklahoma State Board of Behavioral Health Licensure
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Oklahoma private-practice clinicians use superbills for out-of-network (OON) reimbursement and invoices for cash-pay clients. This generator produces either document entirely in the browser: no account, no PHI upload, and a downloadable PDF with rendering credential, Oklahoma license string, NPI, CPT, ICD-10, place of service, and amounts.
Choose Oklahoma defaults when the rendering provider’s authority and the client’s location for the session are Oklahoma (or another state you are lawfully authorized to serve). Match the exact title on your Oklahoma State Board of Behavioral Health Licensure (BBHL) or sister-board record. This page supports documentation workflow only; it is not legal, billing, or tax advice.
License titles that belong on the form
Oklahoma’s independent counseling title is Licensed Professional Counselor (LPC). That is the generator default. Candidate / under-supervision credentials exist while accruing hours—print the candidate or associate title you actually hold, not LPC.
Other titles commonly used on Oklahoma superbills:
- LCSW — Licensed Clinical Social Worker (Oklahoma State Board of Licensed Social Workers—separate from BBHL).
- LMFT — Licensed Marital and Family Therapist (also under BBHL).
- Licensed Psychologist — psychologists licensed by the Oklahoma State Board of Examiners of Psychologists—print the exact title on your license.
Exact credential dropdown strings:
`LPC` · `LCSW` · `LMFT` · `Licensed Psychologist`
BBHL also licenses Licensed Behavioral Practitioners (LBP); if that is your credential, note it accurately for the payer even if it is not in this page’s default dropdown. Match the letters on your wall certificate and the online registry.
Board, license number, lookup
LPC, LMFT, and LBP licenses are issued by the Oklahoma State Board of Behavioral Health Licensure (BBHL) — see oklahoma.gov/behavioralhealth and the BBHL Thentia portal.
Official verification: BBHL Online Registry / licensee search. Copy the license number exactly as shown. The generator hint OK LPC # is a placeholder—replace it with your real BBHL number. Include individual NPI (and group NPI if applicable) plus tax ID as the payer requires.
Social workers and psychologists verify through their own Oklahoma boards—do not confuse BBHL counseling licenses with social work or psychology lookups.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — enacted (not yet operational for privileges). Oklahoma appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational.
- PSYPACT — yes. Oklahoma participates for psychologists with required ASPPB authorizations. Confirm on psypact.gov.
- Social Work Licensure Compact — enacted. Oklahoma is listed on swcompact.org/compact-jurisdictions. Multistate SW licenses are not yet issuing—confirm at swcompact.org.
Verify before you rely on it. Compact membership and privilege systems change.
Telehealth + place of service
Use POS 10 when the patient is at home, or POS 02 for telehealth elsewhere. Add modifier 95 for synchronous telemedicine when the payer’s guide requires it. Do not use POS 11 for a video-only visit.
An Oklahoma LPC license, a live Counseling Compact privilege naming Oklahoma (when operational), or another lawful authorization must cover the client’s location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Oklahoma sales tax is oriented toward tangible personal property and enumerated taxable activities. Professional therapy receipts are generally treated like other professional-service practices: the clinician is typically the consumer of supplies used in practice, and session fees are not measured as taxable retail sales of goods. Oklahoma Tax Commission rules for physicians illustrate this consumer/professional-service pattern (OAC 710:65-19-261).
Leave the invoice sales tax at $0 for clinical services. If you also sell taxable tangible goods apart from incidental items bundled into care, collect tax on those goods. Confirm edge cases with the Oklahoma Tax Commission or a tax professional.

Two worked Oklahoma examples
Oklahoma's counseling paperwork runs through BBHL — the State Board of Behavioral Health Licensure — which also covers LMFTs and Licensed Behavioral Practitioners, while social workers sit on a separate board entirely. The two documents below come from a fictional Tulsa practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Oklahoma invoice
An LPC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Mar 6 | Individual psychotherapy, 45 min | 90834 | 11 | $120.00 | $120.00 |
| Mar 13 | Individual psychotherapy, 45 min | 90834 | 11 | $120.00 | $120.00 |
| Mar 20 | Individual psychotherapy, 45 min | 90834 | 11 | $120.00 | $120.00 |
| Mar 27 | Individual psychotherapy, 45 min | 90834 | 11 | $120.00 | $120.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $480.00 | $480.00 | |||
POS 11 is accurate for office sessions. The tax line stays at $0.00 because Oklahoma generally does not impose sales tax on professional therapy receipts — licensed professionals rendering services are typically consumers of supplies, not retailers of sessions. The credential is LPC, and the license field follows the OK LPC # hint — copy the number exactly as BBHL's online registry displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Norman has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Aug 5 | 90791 | 95 | 10 | F41.1 | $150.00 | $150.00 |
| Aug 12 | 90834 | 95 | 10 | F41.1 | $120.00 | $120.00 |
| Aug 19 | 90834 | 95 | 10 | F41.1 | $120.00 | $120.00 |
| Aug 26 | 90834 | 95 | 10 | F41.1 | $120.00 | $120.00 |
| Total | $510.00 | $510.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LPC, the license number copied verbatim from BBHL's Thentia online registry, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays money already spent — a blank paid column gives the plan nothing to reimburse.
Both parties were in Oklahoma, so the state license sufficed. Oklahoma has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.
Common mistakes on Oklahoma superbills
- Printing LPC under a candidate credential. Candidate and under-supervision statuses are not independent LPC. Print the credential you actually hold and identify the supervisor when the plan asks.
- Verifying a social worker on the wrong board. LCSWs verify through the Oklahoma State Board of Licensed Social Workers — separate from BBHL, which covers LPC, LMFT, and LBP licenses.
- Forgetting LBP exists. BBHL also licenses Licensed Behavioral Practitioners. If that is your credential, print it accurately for the payer even though it is not in the default dropdown.
- Treating enacted compact status as live. Oklahoma is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Oklahoma generally does not tax professional therapy receipts. Leave the line at $0 unless you sell taxable tangible goods.
FAQ
Associate titles in Oklahoma?
If you hold a candidate/under-supervision credential, print that title rather than LPC. Identify the supervisor when a payer asks.

Does Oklahoma tax therapy sessions?
Professional therapy is generally exempt from Oklahoma sales tax. Leave tax at $0 on session-only invoices unless you sell taxable goods.
Can an Oklahoma psychologist use PSYPACT?
Oklahoma participates in PSYPACT. You still need active authorizations—verify on psypact.gov.
What do OON payers want on an Oklahoma superbill?
Name + Oklahoma credential, BBHL (or sister-board) license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Oklahoma State Board of Behavioral Health Licensure (BBHL), the official interstate-compact commissions, and Oklahoma's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
