Arizona superbill and invoice for LPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Arizona selected. Out-of-network clients still need a superbill with your NPI, tax ID, Arizona license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Arizona superbills
Independent counseling in Arizona is commonly written as LPC. Also print the letters you actually hold: LPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Arizona Board of Behavioral Health Examiners. License line hint: AZ LPC-XXXX.
Arizona was in the first wave of Counseling Compact privilege-to-practice states. Psychologists can use PSYPACT into AZ.
Telehealth coding (national rules, used in Arizona)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Arizona
Professional mental health services are generally exempt from Arizona transaction privilege tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Operational / issuing privileges
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
What license number format does Arizona use?
BBHE issues type-prefixed numbers (for example LPC-12345). Print the prefix and number exactly as they appear on your wall license.
What to put on a Arizona superbill
- Your legal name, LPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Arizona license number as printed by the Arizona Board of Behavioral Health Examiners
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Arizona private-practice clinicians need out-of-network (OON) paperwork that matches Arizona Board of Behavioral Health Examiners (BBHE) titles—not a generic “therapist” line that payers cannot map to a license type. This page’s generator builds a therapist invoice or CMS-style superbill PDF in the browser for LPC, LAC, LCSW, LMFT, and Licensed Psychologist credentials. Defaults are tuned for Arizona: credential dropdown, AZ LPC-XXXX license hint (type-prefixed BBHE numbers), and state abbreviation AZ on the form.
An invoice is what you send the client for payment (dates, CPT/HCPCS, charges, amount due). A superbill is the same clinical/billing detail packaged so the client can submit to their insurer for OON reimbursement. Both need accurate rendering-provider name, credential, license number, NPI, diagnosis (ICD-10), procedure codes, place of service, and amounts. Everything runs locally in your browser—no account required and no PHI uploaded to a server for this tool.
License titles that belong on the form
Arizona’s independent counseling license is Licensed Professional Counselor (LPC). Associates hold Licensed Associate Counselor (LAC) and must practice under supervision. Do not print LPC if you hold only LAC.
Independent titles commonly used on Arizona superbills:
- LPC — Licensed Professional Counselor (default on this page)
- LCSW — Licensed Clinical Social Worker
- LMFT — Licensed Marriage and Family Therapist
- Licensed Psychologist — Arizona Board of Psychologist Examiners (separate from BBHE)
Associate / pre-independent:
- LAC — Licensed Associate Counselor
Associates practice under a licensed supervisor. Many commercial OON plans still process a superbill in the associate’s name when the credential is honest (LAC) and the supervisor is identified in notes when the plan asks. Do not upgrade the title on the form.
Exact strings for this generator’s credential dropdown: `LPC`, `LCSW`, `LMFT`, `Licensed Psychologist`, `LAC`.
Board, license number, lookup
Board (counselors, social workers, MFTs, addiction counselors): Arizona Board of Behavioral Health Examiners (BBHE) — bbhe.az.gov.
License verification: BBHE BOARDAL public registry — License Verification Online Registry.
BBHE issues type-prefixed numbers (for example LPC-12345 or LAC-#####). Print the prefix and digits exactly as they appear on your wall license and in BOARDAL. On superbills, a clear “AZ LPC-#####” (or your actual type prefix) next to the credential is what commercial payers expect. Refresh the lookup before you mail or email a first-time OON packet.
Psychologists use the Arizona Board of Psychologist Examiners pathway; put that board’s number on the rendering line if you are billing as a psychologist.
Interstate practice: Counseling Compact / PSYPACT / Social Work Compact
As of retrieval on 2026-09-14:
- Counseling Compact: Arizona is a member and is among the early operational privilege-to-practice states on the official map at counselingcompact.gov/map. Privileges only work when both the home state and the remote state are operational—verify CompactConnect before you rely on a privilege.
- PSYPACT: Arizona is a PSYPACT participating jurisdiction for psychologists (verify the live map at psypact.gov). Authorization holders still need current E.Passport / APIT credentials from the Commission—not home-state licensure alone.
- Social Work Licensure Compact: Arizona has enacted the compact language and appears on swcompact.org/compact-jurisdictions. Multistate SW licenses were not yet being issued as of mid-2026 commission updates—confirm before advertising multistate SW practice.
Verify before you rely on any compact. Maps and privilege systems change. Compact status on this page is a snapshot for invoice defaults, not authorization to practice.
Telehealth + place of service
For video sessions, commercial claims and superbills commonly use:
- Place of service (POS) 10 — telehealth provided in patient’s home
- POS 02 — telehealth provided other than in patient’s home
- Modifier 95 — synchronous telemedicine service via real-time interactive audio and video (when the payer wants it)
Do not use POS 11 (office) for a pure video visit. Arizona’s Counseling Compact operational status and PSYPACT participation create lawful interstate pathways when you actually hold the privilege or authorization. Print the Arizona credential and number (or compact privilege documentation your payer accepts) that authorize the session where the client is located.
Sales tax on therapy invoices
Arizona’s transaction privilege tax (TPT) generally does not apply to professional or personal service occupations when tangible personal property is only an inconsequential element of the transaction (A.R.S. § 42-5061(A)(1); ADOR Model City Tax Code — retail sales: professional services). Session fees for licensed mental health counseling—when the true object is the clinical service—are therefore typically left at $0 tax on the invoice. Separately sold retail goods (workbooks, devices, merch) can be taxable; confirm with ADOR or a tax professional if you sell products.
This is not tax advice.
How to fill the Arizona generator
- Confirm State = AZ (this page’s default).
- Choose your credential (default LPC). Associates select LAC—not LPC.
- Enter your name as rendering provider and AZ LPC-XXXX (or LAC/LCSW/LMFT prefix) exactly as in BOARDAL.
- Add NPI, practice address, EIN/SSN as your payer packet requires, and taxonomy if you use one.
- Enter each date of service, CPT (for example 90834, 90837), ICD-10, units, charges, and amount paid.
- Set POS to 10 or 02 for video; add modifier 95 when the client’s plan expects it.
- For LACs, put the supervisor’s name and license in notes when the insurer asks.
- Download the PDF. Keep a copy; the client submits the superbill to the payer.
No PHI is uploaded by this browser-only flow.
Two worked Arizona examples
Arizona documents stand or fall on one detail more than most: the BBHE license number is type-prefixed — LPC-12345, not a bare digit string — and a payer matching the number against the BOARDAL registry will catch a missing prefix. The two documents below come from a fictional Tucson practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Arizona invoice
An LPC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| May 7 | Individual psychotherapy, 45 min | 90834 | 11 | $140.00 | $140.00 |
| May 14 | Individual psychotherapy, 45 min | 90834 | 11 | $140.00 | $140.00 |
| May 21 | Individual psychotherapy, 45 min | 90834 | 11 | $140.00 | $140.00 |
| May 28 | Individual psychotherapy, 45 min | 90834 | 11 | $140.00 | $140.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $560.00 | $560.00 | |||
POS 11 is correct for office sessions. The tax line reads $0.00 because Arizona's transaction privilege tax does not apply to professional or personal service occupations when tangible property is only an inconsequential element — the true object here is the clinical service, per A.R.S. § 42-5061(A)(1) and ADOR's Model City Tax Code guidance. The credential is LPC, and the license field follows the AZ LPC-XXXX hint — print the prefix and digits exactly as BOARDAL shows them.
Example 2 — a telehealth out-of-network superbill
A second client in Scottsdale has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Jul 2 | 90791 | 95 | 10 | F43.10 | $170.00 | $170.00 |
| Jul 9 | 90834 | 95 | 10 | F43.10 | $140.00 | $140.00 |
| Jul 16 | 90834 | 95 | 10 | F43.10 | $140.00 | $140.00 |
| Jul 23 | 90834 | 95 | 10 | F43.10 | $140.00 | $140.00 |
| Total | $590.00 | $590.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 identifies synchronous audio-video service for plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block shows the rendering name with LPC, the type-prefixed license number copied from BBHE's BOARDAL registry — LPC-##### exactly as the wall license shows it — plus NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays money the client already spent.
Both parties were in Arizona, so the state license sufficed. Arizona is among the early operational Counseling Compact states, which means an Arizona LPC can hold privileges in other live compact states — but only where both states are live on the date of service, and only for the independently licensed. An LAC treating these sessions would print the associate credential, identify the supervisor when the plan asks, and make no compact claim at all.
Common mistakes on Arizona superbills
- Dropping the license-number prefix. BBHE numbers are type-prefixed (LPC-12345, LAC-#####). A bare digit string fails to match the BOARDAL registry — copy the full string, prefix included.
- Printing LPC while holding LAC. Licensed Associate Counselors practice under supervision. Disclose LAC and identify the supervisor when the plan asks; do not print the independent title early.
- Verifying a psychologist on the wrong board. Psychologists are licensed by the Arizona Board of Psychologist Examiners, not BBHE — use that board's number when billing as a psychologist.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a TPT line to session fees. Professional counseling is nontaxable under A.R.S. § 42-5061(A)(1) when property is incidental. Leave tax at $0 unless you separately sell taxable goods.
- Leaving the paid column blank. OON plans reimburse actual expenditures — a superbill without payments gives the adjuster nothing to act on.
FAQ
What license number format does Arizona use?
BBHE issues type-prefixed numbers (for example LPC-12345). Print the prefix and number exactly as they appear on your wall license and BOARDAL.
Can an Arizona LAC issue a superbill?
Yes if you disclose LAC and identify the supervisor when the plan requires it. Do not list LPC until BBHE has issued that independent license.
Is Arizona in the Counseling Compact?
Yes—and it is among the early operational states. Privileges still require both states to be live; check counselingcompact.gov and CompactConnect.
Does Arizona TPT apply to therapy sessions?
Professional counseling fees are generally nontaxable professional services under A.R.S. § 42-5061(A)(1) when property is incidental. Leave tax at $0 unless you sell taxable goods—verify with ADOR.
What do OON payers want on an Arizona superbill?
Rendering name + credential, AZ license with type prefix, NPI, dates of service, CPT, ICD-10, POS, charges, amount paid—plus modifier 95 for telehealth when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Arizona Board of Behavioral Health Examiners, the official interstate-compact commissions, and Arizona's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
