South Dakota superbill and invoice for LPC-MH, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with South Dakota selected. Out-of-network clients still need a superbill with your NPI, tax ID, South Dakota license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on South Dakota superbills
Independent counseling in South Dakota is commonly written as LPC-MH. Also print the letters you actually hold: LPC, LPC-MH, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the South Dakota Board of Examiners for Counselors and Marriage and Family Therapists. License line hint: SD LPC-MH #.
South Dakota has LPC and the mental-health designation LPC-MH. Private-practice clinicians billing OON typically hold LPC-MH.
Telehealth coding (national rules, used in South Dakota)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in South Dakota
Licensed health services, including many psychotherapy services, are treated as exempt under South Dakota rules. Confirm if you also sell non-clinical products. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
LPC or LPC-MH?
Use LPC-MH if that is the credential that authorizes diagnosis and independent mental health practice for you.
What to put on a South Dakota superbill
- Your legal name, LPC-MH or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- South Dakota license number as printed by the South Dakota Board of Examiners for Counselors and Marriage and Family Therapists
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
South Dakota private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, South Dakota license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select South Dakota defaults when your rendering authority and the client's location for the session are South Dakota (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
South Dakota has LPC and the mental-health designation LPC-MH. Private-practice clinicians billing OON for diagnosis and independent mental health practice typically hold LPC-MH—that is the generator default. Print LPC only if that is the credential authorizing your services.
Other titles commonly used on South Dakota superbills:
- LPC — print the exact letters on your South Dakota license/board record.
- LCSW — print the exact letters on your South Dakota license/board record.
- LMFT — print the exact letters on your South Dakota license/board record.
- Licensed Psychologist — print the exact letters on your South Dakota license/board record.
Exact credential dropdown strings:
`LPC` · `LPC-MH` · `LCSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Counselor and MFT licenses are issued by the South Dakota Board of Examiners for Counselors and Marriage and Family Therapists. Verify through the board's official DSS licensing pages and copy the number exactly.
Official verification: https://dss.sd.gov/licensingboards/counselors/counselors.aspx. Copy the license number exactly as shown. The generator hint SD LPC-MH # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — enacted (not yet operational for privileges). South Dakota appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
- PSYPACT — yes. South Dakota participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
- Social Work Licensure Compact — enacted. South Dakota is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A South Dakota license, a live Counseling Compact privilege naming South Dakota (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
South Dakota taxes many services by default but provides exemptions for certain licensed health services. Psychotherapy provided by appropriately licensed clinicians is commonly treated as exempt—confirm your fact pattern with the South Dakota Department of Revenue. Leave tax at $0 on clinical invoices unless you sell taxable non-clinical products.

Two worked South Dakota examples
South Dakota's paperwork hinges on a hyphen: LPC-MH — Licensed Professional Counselor with the Mental Health designation — is the credential that authorizes independent diagnosis and mental-health practice, and plain LPC is a different scope. The two documents below come from a fictional Sioux Falls practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay South Dakota invoice
An LPC-MH sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| May 5 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| May 12 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| May 19 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| May 26 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $500.00 | $500.00 | |||
POS 11 is accurate for office sessions. The tax line stays at $0.00 because licensed health services — including many psychotherapy services — are treated as exempt under South Dakota rules. The credential is LPC-MH, hyphen included, and the license field follows the SD LPC-MH # hint — copy the number exactly as the Board of Examiners' DSS licensing pages display it.
Example 2 — a telehealth out-of-network superbill
A second client in Rapid City has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Jul 7 | 90791 | 95 | 10 | F41.0 | $155.00 | $155.00 |
| Jul 14 | 90834 | 95 | 10 | F41.0 | $125.00 | $125.00 |
| Jul 21 | 90834 | 95 | 10 | F41.0 | $125.00 | $125.00 |
| Jul 28 | 90834 | 95 | 10 | F41.0 | $125.00 | $125.00 |
| Total | $530.00 | $530.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LPC-MH — the mental-health designation is what authorizes the independent clinical work this document describes — plus the license number copied from the DSS licensing pages, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures.
Both parties were in South Dakota, so the state license sufficed. South Dakota has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.
Common mistakes on South Dakota superbills
- Dropping the -MH designation. LPC-MH is the credential authorizing independent mental-health practice; plain LPC is a different scope. Print the full designation — hyphen included — as it appears on the license.
- Printing LPC-MH when you hold LPC. The designation follows the license, not the setting. Print the credential that authorizes your services and identify the supervisor when the plan asks.
- Verifying on the wrong site. Counselor and MFT licenses verify through the Board of Examiners' DSS licensing pages — use that official channel rather than a third-party directory.
- Treating enacted compact status as live. South Dakota is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Licensed health services are treated as exempt under South Dakota rules. Leave the line at $0 on therapy-only invoices — and confirm if you also sell non-clinical products.

Why these sources matter
Every factual claim on this page traces back to a primary source: South Dakota Board of Examiners for Counselors and Marriage and Family Therapists, the official interstate-compact commissions, and South Dakota's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
