New Hampshire superbill and invoice for LCMHC, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with New Hampshire selected. Out-of-network clients still need a superbill with your NPI, tax ID, New Hampshire license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on New Hampshire superbills

Independent counseling in New Hampshire is commonly written as LCMHC. Also print the letters you actually hold: LCMHC, LICSW, LMFT, Licensed Psychologist. The board to match your number against is the New Hampshire Board of Mental Health Practice. License line hint: NH LCMHC #.

New Hampshire counselors are Licensed Clinical Mental Health Counselors (LCMHC).

Telehealth coding (national rules, used in New Hampshire)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in New Hampshire

New Hampshire has no general sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

Any tax line on a New Hampshire invoice?

No statewide sales tax. Leave tax at $0 unless you have a specific local obligation.

What to put on a New Hampshire superbill

  • Your legal name, LCMHC or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • New Hampshire license number as printed by the New Hampshire Board of Mental Health Practice
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

New Hampshire private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, New Hampshire license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select New Hampshire defaults when your rendering authority and the client's location for the session are New Hampshire (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

New Hampshire counselors are Licensed Clinical Mental Health Counselors (LCMHC). Print LCMHC—not LPC. Independent clinical social workers commonly use LICSW. MFTs use LMFT; psychologists use Licensed Psychologist.

Other titles commonly used on New Hampshire superbills:

  • LICSW — print the exact letters on your New Hampshire license/board record.
  • LMFT — print the exact letters on your New Hampshire license/board record.
  • Licensed Psychologist — print the exact letters on your New Hampshire license/board record.

Exact credential dropdown strings:

`LCMHC` · `LICSW` · `LMFT` · `Licensed Psychologist`

Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.

Board, license number, lookup

Licenses are issued by the New Hampshire Board of Mental Health Practice under OPLC. Verify via New Hampshire's official license verification tool and copy the number exactly.

Official verification: https://forms.nh.gov/licenseverification/. Copy the license number exactly as shown. The generator hint NH LCMHC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — enacted (not yet operational for privileges). New Hampshire appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
  • PSYPACT — yes. New Hampshire participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
  • Social Work Licensure Compact — enacted. New Hampshire is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.

Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.

Telehealth + place of service

For video psychotherapy superbills, use the place-of-service codes payers expect:

  • POS 10 — patient at home
  • POS 02 — telehealth other than home
  • Modifier 95 — when the plan wants synchronous telemedicine marked

Do not code a video-only session as POS 11. A New Hampshire license, a live Counseling Compact privilege naming New Hampshire (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

New Hampshire has no general statewide sales tax. Leave the invoice tax line at $0 for clinical services. See New Hampshire Department of Revenue Administration.

Two worked New Hampshire examples

New Hampshire's paperwork is light on tax — the state has no general sales tax — but particular about credentials: the counseling title is LCMHC, and verification runs through OPLC's online license-verification tool. The two documents below come from a fictional Concord practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.

Example 1 — an in-office private-pay New Hampshire invoice

An LCMHC sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
May 6Individual psychotherapy, 60 min9083711$155.00$155.00
May 13Individual psychotherapy, 60 min9083711$155.00$155.00
May 20Individual psychotherapy, 60 min9083711$155.00$155.00
May 27Individual psychotherapy, 60 min9083711$155.00$155.00
Sales tax (see below)$0.00
Total$620.00$620.00

POS 11 is correct for office sessions. The tax line reads $0.00 for the simplest possible reason — New Hampshire has no general sales tax at all, so there is nothing to compute. The credential is LCMHC, Licensed Clinical Mental Health Counselor — not LPC — and the license field follows the NH LCMHC # hint: copy the number exactly as the Board of Mental Health Practice's verification tool displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Portsmouth has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Jul 8907919510F32.1$185.00$185.00
Jul 15908379510F32.1$155.00$155.00
Jul 22908379510F32.1$155.00$155.00
Jul 29908379510F32.1$155.00$155.00
Total$650.00$650.00

The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous audio-video service for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.

The provider block carries the rendering name with LCMHC, the license number copied verbatim from OPLC's License Verification, NPI, practice address, and tax ID. If the rendering clinician is an independent clinical social worker rather than a counselor, the credential is LICSW — a separate board, so verify it through that board's lookup rather than the OPLC tool. The paid column is populated because OON reimbursement repays actual expenditures.

That last detail matters more than it looks. An out-of-network claim reimburses money the client has already spent, and the OPLC record is exactly what the payer's verification desk checks — the printed credential type, the number, and an active status all have to agree with it. In a group practice the superbill lists the individual rendering clinician while the tax ID stays the practice's; when the plan asks for a supervising clinician for supervised-rendered care, identify the supervisor in the notes rather than in the signature block.

Both parties were in New Hampshire, so the state license sufficed. New Hampshire has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.

Common mistakes on New Hampshire superbills

  • Writing "LPC" instead of "LCMHC". New Hampshire's counseling title is Licensed Clinical Mental Health Counselor. Out-of-state abbreviations fail to match the Board of Mental Health Practice record.
  • Printing LCSW when the license says LICSW. New Hampshire's independent clinical social work credential is LICSW — print the exact letters on the license.
  • Printing a supervised status as LCMHC. Associate and provisional statuses are not the independent title — print the exact credential held and identify the supervisor when the plan asks.
  • Treating enacted compact status as live. New Hampshire is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments gives the adjuster nothing to act on.

FAQ

New Hampshire participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.

What do OON payers want on a New Hampshire superbill?

Name + New Hampshire credential (LCMHC or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.

Why these sources matter

Every factual claim on this page traces back to a primary source: New Hampshire Board of Mental Health Practice (OPLC), the official interstate-compact commissions, and New Hampshire's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
