Iowa superbill and invoice for LMHC, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Iowa selected. Out-of-network clients still need a superbill with your NPI, tax ID, Iowa license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Iowa superbills

Independent counseling in Iowa is commonly written as LMHC. Also print the letters you actually hold: LMHC, LISW, LMFT, Licensed Psychologist. The board to match your number against is the Iowa Board of Behavioral Science. License line hint: IA LMHC #.

Iowa counselors use LMHC; independent social workers often use LISW. Confirm current PSYPACT effective date before advertising interstate psychology.

Telehealth coding (national rules, used in Iowa)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Iowa

Professional mental health services are generally exempt from Iowa sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Pending / verify current status
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

LISW or LCSW in Iowa?

Iowa's clinical social work title is commonly LISW. Print the letters exactly as they appear on your license card.

What to put on a Iowa superbill

  • Your legal name, LMHC or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Iowa license number as printed by the Iowa Board of Behavioral Science
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Iowa private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Iowa license number, NPI, CPT, ICD-10, place of service, and payment lines.

Select Iowa defaults when your rendering authority and the client's location for the session are Iowa (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.

License titles that belong on the form

Iowa counselors use Licensed Mental Health Counselor (LMHC) (temporary/T-LMHC titles may apply while supervised—print the exact board string). Independent clinical social workers commonly use LISW rather than LCSW. MFTs use LMFT; psychologists use Licensed Psychologist.

Other titles commonly used on Iowa superbills:

  • LISW — print the exact letters on your Iowa license/board record.
  • LMFT — print the exact letters on your Iowa license/board record.
  • Licensed Psychologist — print the exact letters on your Iowa license/board record.

Exact credential dropdown strings:

`LMHC` · `LISW` · `LMFT` · `Licensed Psychologist`

Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.

Board, license number, lookup

Behavioral health licenses are administered through Iowa's professional licensing structure (Board of Behavioral Health Professionals / DIAL pathways). Verify via the official Iowa professional license portal and copy the number exactly as shown.

Official verification: https://ibplicense.iowa.gov/PublicPortal/Iowa/IBPL/common/index.jsp. Copy the license number exactly as shown. The generator hint IA LMHC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Snapshot for 2026-09-14 (always re-check official maps):

  • Counseling Compact — enacted (not yet operational for privileges). Iowa appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
  • PSYPACT — pending. Iowa passed PSYPACT legislation (psypact.gov announced July 1, 2026; effective one year later). Do not advertise interstate psychology under PSYPACT until Iowa is active on the official map and you hold authorizations.
  • Social Work Licensure Compact — enacted. Iowa is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.

Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.

Telehealth + place of service

For video psychotherapy superbills, use the place-of-service codes payers expect:

  • POS 10 — patient at home
  • POS 02 — telehealth other than home
  • Modifier 95 — when the plan wants synchronous telemedicine marked

Do not code a video-only session as POS 11. A Iowa license, a live Counseling Compact privilege naming Iowa (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.

Sales tax on therapy invoices

Iowa sales tax generally applies to tangible personal property and enumerated taxable services. Professional mental health counseling/psychotherapy is generally exempt. Leave tax at $0 on clinical invoices. Confirm with the Iowa Department of Revenue if you sell goods.

How to fill the Iowa generator

  1. Choose Superbill or Invoice.
  2. Set jurisdiction to IA.
  3. Enter rendering name; select LMHC (default) or another credential you currently hold from the dropdown.
  4. Complete practice demographics, NPI, tax ID, and Iowa board license number.
  5. Add client member ID / DOB only if needed for the OON claim.
  6. Enter DOS, CPT, ICD-10, units, fees, and payments.
  7. Telehealth: POS 10 or 02; modifier 95 when required.
  8. Keep sales tax aligned with Iowa rules (see tax section)—usually $0 on therapy-only fees unless local advice says otherwise.
  9. Download the PDF locally—nothing is uploaded.

Two worked Iowa examples

Iowa's paperwork quirks are quiet but real: the counseling credential is LMHC rather than LPC, and independent clinical social workers carry LISW — a different abbreviation than most states use. The two documents below come from a fictional Des Moines practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.

Example 1 — an in-office private-pay Iowa invoice

An LMHC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
Mar 6Individual psychotherapy, 45 min9083411$130.00$130.00
Mar 13Individual psychotherapy, 45 min9083411$130.00$130.00
Mar 20Individual psychotherapy, 45 min9083411$130.00$130.00
Mar 27Individual psychotherapy, 45 min9083411$130.00$130.00
Sales tax (see below)$0.00
Total$520.00$520.00

POS 11 is correct for office sessions. The tax line reads $0.00 because professional mental health services are generally exempt from Iowa sales tax — therapy-only invoices carry no taxable line. The credential is LMHC, Iowa's counseling title, and the license field follows the IA LMHC # hint — copy the number exactly as the official Iowa professional license portal displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Cedar Rapids has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Sep 3907919510F41.1$160.00$160.00
Sep 10908349510F41.1$130.00$130.00
Sep 17908349510F41.1$130.00$130.00
Sep 24908349510F41.1$130.00$130.00
Total$550.00$550.00

The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.

The provider block shows the rendering name with LMHC, the license number copied from the Iowa licensing portal, NPI, practice address, and tax ID. If the rendering clinician were an independent clinical social worker, the credential would be LISW — Iowa's title, not the LCSW most states use. The paid column is populated because OON reimbursement repays money already spent.

Both parties were in Iowa, so the state license sufficed. Iowa has enacted the Counseling Compact, and privileges require both home and remote states to be live on the Commission map before reliance. Iowa's PSYPACT status is pending — enacted legislation that has not yet made the state active on the official map — so psychologists should verify before counting on it for either direction of practice.

Common mistakes on Iowa superbills

  • Writing "LPC" instead of "LMHC". Iowa's counseling title is Licensed Mental Health Counselor. Out-of-state abbreviations fail to match the licensing record and can slow payer verification.
  • Printing LCSW when the license says LISW. Iowa's independent clinical social work credential is LISW — a different abbreviation than most states. Print the exact letters on the license.
  • Printing LMHC while holding a temporary title. Temporary or T-LMHC statuses are supervised credentials — print the exact board string and identify the supervisor when the plan asks.
  • Treating PSYPACT as live. Iowa's participation is pending until the official map shows it active and you hold ASPPB authorization — verify before interstate psychology work.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Adding a sales-tax line. Professional mental health services are generally exempt from Iowa sales tax. Leave the line at $0 unless you sell taxable goods.

Why these sources matter

Every factual claim on this page traces back to a primary source: Iowa Board of Behavioral Health Professionals, the official interstate-compact commissions, and Iowa's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
