Tennessee superbill and invoice for LPC-MHSP, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Tennessee selected. Out-of-network clients still need a superbill with your NPI, tax ID, Tennessee license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Tennessee superbills

Independent counseling in Tennessee is commonly written as LPC-MHSP. Also print the letters you actually hold: LPC, LPC-MHSP, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Tennessee Board for Licensed Professional Counselors, Marital and Family Therapists and Clinical Pastoral Therapists. License line hint: TN LPC-MHSP #.

Tennessee independent mental health counselors typically hold LPC-MHSP (Mental Health Service Provider). Compact privileges have begun in Tennessee — verify before use.

Telehealth coding (national rules, used in Tennessee)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Tennessee

Professional mental health services are generally exempt from Tennessee sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Operational / issuing privileges
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

Is LPC enough in Tennessee?

For diagnosing and independent mental health practice, payers usually expect LPC-MHSP. Match your certificate.

What to put on a Tennessee superbill

  • Your legal name, LPC-MHSP or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Tennessee license number as printed by the Tennessee Board for Licensed Professional Counselors, Marital and Family Therapists and Clinical Pastoral Therapists
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Tennessee private-practice clinicians prepare invoices for cash-pay clients and superbills for out-of-network (OON) claims. This generator builds either PDF in the browser with Tennessee defaults: LPC-MHSP as the primary independent counseling title, an LPC option when that is the certificate held, and fields for NPI, CPT, ICD-10, place of service, and payments. No account and no PHI upload.

Use Tennessee defaults when your Tennessee authority (or compact privilege) and the client’s location for the session authorize care. Print the designation on your Tennessee Department of Health board certificate. This page is not legal, billing, coding, insurance, or tax advice.

License titles that belong on the form

Tennessee independent mental health counselors typically hold Licensed Professional Counselor – Mental Health Service Provider (LPC-MHSP). That designation is the default on this generator because payers and board materials treat MHSP as the clinical independent counseling pathway for diagnosing and treating mental health conditions. Tennessee also issues LPC without MHSP; those scopes differ—match your certificate.

Other titles commonly used on Tennessee superbills:

  • LCSW — Licensed Clinical Social Worker
  • LMFT — Licensed Marital and Family Therapist (Tennessee board nomenclature)
  • Licensed Psychologist — psychology license under Tennessee psychology regulation

Exact credential dropdown strings:

`LPC` · `LPC-MHSP` · `LCSW` · `LMFT` · `Licensed Psychologist`

Do not print LPC-MHSP if your wall certificate and public license record show LPC only. Temporary licenses and supervised statuses are not independent MHSP credentials. Put supervisor information in notes when required.

Board, license number, lookup

Professional counselors, marital and family therapists, and clinical pastoral therapists are licensed by the Tennessee Board for Licensed Professional Counselors, Marital and Family Therapists and Clinical Pastoral Therapists under the Tennessee Department of Health. Start from the board’s official licensure page for applications, statutes, and verification pathways.

Copy the license number and designation exactly as the Department of Health record shows them. The generator hint TN LPC-MHSP # is a reminder for independent mental health counselors; other professions paste their own Tennessee license string.

Include Type 1 NPI (Type 2 for groups), tax ID, and practice address. Taxonomy codes belong in notes or electronic claims when the plan asks.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Counseling Compact — operational. Tennessee went live for privileges (Commission home page notes Tennessee live as of September 1, 2026). LPC-MHSPs licensed by Tennessee who live in Tennessee may apply for privileges in other live compact states; counselors from other live states may obtain privileges to practice in Tennessee. Associates and non-independent ranks are not eligible. Verify the live map on counselingcompact.gov for every interstate date of service.

PSYPACT — yes. Tennessee participates for psychologists with required ASPPB authorizations. PSYPACT does not cover counselor or social-work practice.

Social Work Licensure Compact — enacted (not yet operational for multistate licenses). Tennessee appears on the official Social Work Compact jurisdictions list. Multistate licenses are not yet issued—confirm at swcompact.org.

Telehealth + place of service

Use POS 10 for patient-home telehealth or POS 02 for telehealth elsewhere, plus modifier 95 when required by the payer. POS 11 is for in-person office services only.

Interstate telehealth still needs lawful authority in the client’s state—Tennessee license, live Counseling Compact privilege, PSYPACT for psychologists, or another lawful pathway. Print the credential that authorizes that encounter.

Sales tax on therapy invoices

Tennessee sales and use tax centers on tangible personal property and enumerated taxable services. Professional mental health counseling and psychotherapy session fees are generally not billed as a sales-tax line on a therapy invoice. Leave sales tax at $0 for ordinary clinical fees.

If you sell taxable goods or other taxable services, follow Tennessee Department of Revenue rules for those lines. Tennessee also has separate business-tax concepts for certain service categories; those are not the same as adding retail sales tax to a client superbill. Confirm edge cases with Revenue or a tax professional.

Documentation tips for Tennessee OON claims

Tennessee payers and EAPs frequently distinguish LPC from LPC-MHSP on credential checks. If your certificate shows Mental Health Service Provider, put LPC-MHSP on the rendering line every time—abbreviating to LPC can trigger unnecessary verification delays.

CompactConnect privileges for Tennessee-home LPC-MHSPs (and privileges into Tennessee from other live states) should be reflected in notes with the privilege number when the encounter relies on compact authority. Home-state license expiration and privilege expiration can diverge after renewals—follow Commission guidance to renew privileges after the home license updates in the data system.

For marital and family therapists and clinical pastoral therapists licensed by the same board family, keep the profession title accurate. Superbills that mix LMFT services with an LPC-MHSP credential string create avoidable mismatches.

Two worked Tennessee examples

Tennessee's paperwork turns on a suffix that carries legal weight: LPC-MHSP — Licensed Professional Counselor with Mental Health Service Provider designation — is the credential payers and board materials treat as the independent clinical pathway, and plain LPC is a different scope. The two documents below come from a fictional Nashville practice. All identifiers are placeholders.

Example 1 — an in-office private-pay Tennessee invoice

An LPC-MHSP sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
Apr 7Individual psychotherapy, 60 min9083711$145.00$145.00
Apr 14Individual psychotherapy, 60 min9083711$145.00$145.00
Apr 21Individual psychotherapy, 60 min9083711$145.00$145.00
Apr 28Individual psychotherapy, 60 min9083711$145.00$145.00
Sales tax (see below)$0.00
Total$580.00$580.00

POS 11 is correct for office sessions. The tax line stays at $0.00 because Tennessee's sales and use tax does not generally apply to professional mental health counseling session fees — allied health-type professional services sit outside taxable retail services. The credential is LPC-MHSP, and the license field follows the TN LPC-MHSP # hint — copy the number and designation exactly as the Department of Health record shows them.

Example 2 — a telehealth out-of-network superbill

A second client in Knoxville has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Aug 4907919510F33.1$175.00$175.00
Aug 11908379510F33.1$145.00$145.00
Aug 18908379510F33.1$145.00$145.00
Aug 25908379510F33.1$145.00$145.00
Total$610.00$610.00

The client was at home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous audio-video service for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.

The provider block carries the rendering name with LPC-MHSP — the designation is what authorizes the diagnosing-and-treating work this document describes — plus the Department of Health license number, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures.

Both parties were in Tennessee, so the state license sufficed. Tennessee is among the operational Counseling Compact states — privileges are actually issuing — so a Tennessee LPC-MHSP can hold privileges in other live compact states. The privilege must still name the remote state, and temporary or supervised statuses are not privilege credentials.

Common mistakes on Tennessee superbills

  • Dropping the -MHSP designation. LPC-MHSP is the clinical independent pathway for diagnosing and treating mental health conditions. Plain LPC is a different scope — print the full designation as your certificate shows it.
  • Printing LPC-MHSP when the license says LPC. The designation follows the license, not the work. If your certificate and public record show LPC only, print LPC.
  • Printing a temporary license as independent. Temporary and supervised statuses are not MHSP credentials — print the status you hold and put supervisor information in notes when required.
  • Assuming compact privileges cover supervisees. Tennessee's operational Counseling Compact extends to independently licensed counselors — supervised statuses are not privilege credentials.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Adding a sales-tax line. Professional counseling fees are outside Tennessee's taxable retail-services base. Leave the line at $0 unless you sell taxable goods.

FAQ

Is LPC enough in Tennessee?

For diagnosing and independent mental health practice, payers usually expect LPC-MHSP. Match your certificate—do not invent MHSP on the PDF.

Is Tennessee Counseling Compact operational?

Yes—Tennessee is among the live privilege states. Confirm the remote state is also live before interstate care.

Does Tennessee tax therapy sessions?

Ordinary professional mental health session fees are generally left at $0 sales tax. Confirm separately sold goods with the Department of Revenue.

What do OON payers want?

Name, LPC-MHSP/LPC/LCSW/etc., NPI, Tennessee license number, address, DOS, CPT, ICD-10, POS, charges, and amount paid.

Clinical pastoral therapists?

Tennessee licenses clinical pastoral therapists under the same board family. If that is your credential, print the exact board title in notes or use the closest dropdown string only when it matches—do not mislabel as LPC-MHSP.

Why these sources matter

Every factual claim on this page traces back to a primary source: Tennessee Board for Licensed Professional Counselors, Marital and Family Therapists and Clinical Pastoral Therapists, the official interstate-compact commissions, and Tennessee's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
