Idaho superbill and invoice for LCPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Idaho selected. Out-of-network clients still need a superbill with your NPI, tax ID, Idaho license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Idaho superbills
Independent counseling in Idaho is commonly written as LCPC. Also print the letters you actually hold: LPC, LCPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Idaho Licensing Board of Professional Counselors and MFTs. License line hint: ID LCPC #.
Idaho is two-tier: LPC and the independent clinical title LCPC. Most OON superbills from private-practice clinicians should use LCPC.
Telehealth coding (national rules, used in Idaho)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Idaho
Professional counseling is generally not subject to Idaho sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Not a member
- PSYPACT (psychologists): Active
- Social Work Compact: Not a member
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Should I put LPC or LCPC?
Use the highest credential you currently hold. If you are independently licensed, that is LCPC in Idaho.
What to put on a Idaho superbill
- Your legal name, LCPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Idaho license number as printed by the Idaho Licensing Board of Professional Counselors and MFTs
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Idaho private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Idaho license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select Idaho defaults when your rendering authority and the client's location for the session are Idaho (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
Idaho is two-tier: LPC and the independent clinical title Licensed Clinical Professional Counselor (LCPC). Most OON superbills from private-practice clinicians should use LCPC when that is the license held. Print LPC only if that is your current credential. Idaho also maintains telehealth registration pathways—that registration is separate from a full license.
Other titles commonly used on Idaho superbills:
- LPC — print the exact letters on your Idaho license/board record.
- LCSW — print the exact letters on your Idaho license/board record.
- LMFT — print the exact letters on your Idaho license/board record.
- Licensed Psychologist — print the exact letters on your Idaho license/board record.
Exact credential dropdown strings:
`LPC` · `LCPC` · `LCSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Counselor and MFT licenses are issued by the Idaho Licensing Board of Professional Counselors and Marriage and Family Therapists under DOPL. Verify through DOPL online services and copy the number exactly.
Official verification: https://dopl.idaho.gov/landing/. Copy the license number exactly as shown. The generator hint ID LCPC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — no. Idaho is not listed as a member jurisdiction on counselingcompact.gov/map/compact-states. Do not rely on compact privileges to counsel clients located in Idaho.
- PSYPACT — yes. Idaho participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
- Social Work Licensure Compact — no at retrieval (Idaho not listed on swcompact.org/compact-jurisdictions). Do not advertise multistate SW practice via the compact until the official map includes Idaho.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A Idaho license, a live Counseling Compact privilege naming Idaho (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Idaho sales tax generally applies to retail sales of tangible personal property and certain taxable services. Professional counseling and psychotherapy session fees are generally not subject to Idaho sales tax. Leave tax at $0 on clinical invoices. Confirm with the Idaho State Tax Commission if you sell goods or provide enumerated taxable services.

Two worked Idaho examples
Idaho is a two-tier state — LPC and the independent clinical LCPC — and most private-practice superbill rejections trace back to printing the wrong tier or to assuming a compact pathway that does not exist. The two documents below come from a fictional Boise practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Idaho invoice
An LCPC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| May 6 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| May 13 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| May 20 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| May 27 | Individual psychotherapy, 45 min | 90834 | 11 | $125.00 | $125.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $500.00 | $500.00 | |||
POS 11 is accurate for office sessions. The tax line stays at $0.00 because professional counseling is generally not subject to Idaho sales tax — therapy-only invoices carry no taxable line. The credential is LCPC, Idaho's independent clinical counseling title, and the license field follows the ID LCPC # hint — copy the number exactly as DOPL's online services display it.
Example 2 — a telehealth out-of-network superbill
A second client in Coeur d'Alene has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Aug 5 | 90791 | 95 | 10 | F43.10 | $155.00 | $155.00 |
| Aug 12 | 90834 | 95 | 10 | F43.10 | $125.00 | $125.00 |
| Aug 19 | 90834 | 95 | 10 | F43.10 | $125.00 | $125.00 |
| Aug 26 | 90834 | 95 | 10 | F43.10 | $125.00 | $125.00 |
| Total | $530.00 | $530.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous audio-video service for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LCPC — the independent clinical tier, which is what most OON payers expect from an Idaho private practice — plus the DOPL license number, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays money already spent.
Both parties were in Idaho, so the state license sufficed — and that simplicity matters here, because Idaho is not a Counseling Compact member. Out-of-state counselors reaching Idaho clients cannot rely on a compact privilege; Idaho does maintain telehealth registration pathways, but a registration is separate from a full license and is not something a superbill field can substitute for. Idaho does participate in PSYPACT, though that pathway covers psychologists only.
Common mistakes on Idaho superbills
- Mixing up LPC and LCPC. LCPC is the independent clinical license; LPC is the other tier. Independent private-practice superbills should show LCPC when that is the license held — print LPC only if that is your actual credential.
- Assuming a compact covers Idaho. Idaho is not a Counseling Compact member. Telehealth into Idaho from another state needs an Idaho license or the state's separate telehealth registration pathway — not a home-state license alone.
- Confusing telehealth registration with licensure. Idaho's telehealth registration is separate from a full license. Do not print a license credential you do not hold on the strength of a registration.
- Guessing the license number. Copy the exact string from DOPL's online services rather than reconstructing a format — the payer verifies against that record.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Professional counseling is generally not subject to Idaho sales tax. Leave the line at $0 unless you sell taxable goods.
FAQ
Should I put LPC or LCPC?
Use the highest credential you currently hold. If you are independently licensed for clinical practice, that is LCPC in Idaho.
Is Idaho in the Counseling Compact?
No—Idaho is outside the Counseling Compact. Telehealth or in-person practice into Idaho generally requires a Idaho license (or another lawful authorization).
Does Idaho tax therapy sessions?
Professional counseling is generally not subject to Idaho sales tax. Leave tax at $0 on session-only invoices unless you sell taxable goods.
Can a Idaho psychologist use PSYPACT?
Idaho participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.
What do OON payers want on a Idaho superbill?
Name + Idaho credential (LCPC or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Idaho Licensing Board of Professional Counselors and Marriage and Family Therapists (DOPL), the official interstate-compact commissions, and Idaho's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
