Montana superbill and invoice for LCPC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Montana selected. Out-of-network clients still need a superbill with your NPI, tax ID, Montana license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Montana superbills
Independent counseling in Montana is commonly written as LCPC. Also print the letters you actually hold: LCPC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Montana Board of Behavioral Health. License line hint: MT LCPC #.
Montana independent counselors use LCPC. PSYPACT participation should be verified for the current year before interstate psychology work.
Telehealth coding (national rules, used in Montana)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Montana
Montana has no statewide sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Not a member
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Does Montana tax sessions?
There is no state sales tax. Do not add a tax line unless a local jurisdiction you actually collect for requires it.
What to put on a Montana superbill
- Your legal name, LCPC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Montana license number as printed by the Montana Board of Behavioral Health
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Montana private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Montana license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select Montana defaults when your rendering authority and the client's location for the session are Montana (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
Montana independent counselors use Licensed Clinical Professional Counselor (LCPC). Print LCPC (or the exact supervised/associate string if that is what you hold). Social workers use LCSW tiers as licensed; MFTs use LMFT; psychologists use Licensed Psychologist.
Other titles commonly used on Montana superbills:
- LCSW — print the exact letters on your Montana license/board record.
- LMFT — print the exact letters on your Montana license/board record.
- Licensed Psychologist — print the exact letters on your Montana license/board record.
Exact credential dropdown strings:
`LCPC` · `LCSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Licenses are issued by the Montana Board of Behavioral Health. Verify through Montana's official professional licensing lookup (ebiz/POL) and copy the number exactly.
Official verification: https://ebiz.mt.gov/pol/. Copy the license number exactly as shown. The generator hint MT LCPC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — enacted (not yet operational for privileges). Montana appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
- PSYPACT — yes. Montana participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
- Social Work Licensure Compact — no at retrieval (Montana not listed on swcompact.org/compact-jurisdictions). Do not advertise multistate SW practice via the compact until the official map includes Montana.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A Montana license, a live Counseling Compact privilege naming Montana (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Montana has no statewide sales tax. Do not add a state sales-tax line on therapy invoices. Limited local option or resort taxes may exist in some areas—only collect if you have a documented local obligation. See Montana Department of Revenue.

Two worked Montana examples
Montana's paperwork is spare in one respect — the state levies no general sales tax — and specific in another: the independent counseling title is LCPC, verified through the state's ebiz professional-licensing portal. The two documents below come from a fictional Missoula practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Montana invoice
An LCPC sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Jun 10 | Individual psychotherapy, 60 min | 90837 | 11 | $150.00 | $150.00 |
| Jun 17 | Individual psychotherapy, 60 min | 90837 | 11 | $150.00 | $150.00 |
| Jun 24 | Individual psychotherapy, 60 min | 90837 | 11 | $150.00 | $150.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $450.00 | $450.00 | |||
POS 11 is correct for office sessions. The tax line reads $0.00 — not because counseling earned an exemption, but because Montana has no statewide sales tax at all. Local resort and jurisdiction taxes exist in a few communities, so the $0 reflects that this practice has no local collection duty — confirm yours if you operate somewhere with a resort tax. The credential is LCPC, and the license field follows the MT LCPC # hint — copy the number exactly as the Board of Behavioral Health's licensing lookup displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Bozeman has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Sep 9 | 90791 | 95 | 10 | F41.0 | $180.00 | $180.00 |
| Sep 16 | 90837 | 95 | 10 | F41.0 | $150.00 | $150.00 |
| Sep 23 | 90837 | 95 | 10 | F41.0 | $150.00 | $150.00 |
| Sep 30 | 90837 | 95 | 10 | F41.0 | $150.00 | $150.00 |
| Total | $630.00 | $630.00 | ||||
The client was at home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.
The provider block carries the rendering name with LCPC, the license number copied from the ebiz.mt.gov professional-licensing lookup, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures — a blank paid column gives the plan nothing to reimburse.
Both parties were in Montana, so the state license sufficed. Montana has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.
Common mistakes on Montana superbills
- Printing LPC when the license says LCPC. Montana's independent counseling title is Licensed Clinical Professional Counselor. Match the exact letters on the Board of Behavioral Health record.
- Printing a supervised status as LCPC. Supervised or associate-level credentials are not the independent title — print what you hold and identify the supervisor when the plan asks.
- Assuming no tax means no check. Montana has no statewide sales tax, but local resort and jurisdiction taxes exist — confirm your community's rules before treating the tax line as automatic.
- Treating enacted compact status as live. Montana is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments gives the adjuster nothing to act on.
FAQ
Does Montana tax sessions?
There is no state sales tax. Do not add a tax line unless a local jurisdiction you actually collect for requires it.
Is Montana in the Counseling Compact?
Montana has enacted the compact but is not among the live/operational states as of this retrieval. Privileges work only when both home and remote states are live—check counselingcompact.gov.
Any tax line on a Montana invoice?
No statewide sales tax. Leave tax at $0 on session invoices unless a specific local rule applies.
Can a Montana psychologist use PSYPACT?
Montana participates in PSYPACT. You still need active authorizations and must follow Commission rules—verify on psypact.gov.
What do OON payers want on a Montana superbill?
Name + Montana credential (LCPC or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Montana Board of Behavioral Health, the official interstate-compact commissions, and Montana's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
