Rhode Island superbill and invoice for LMHC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Rhode Island selected. Out-of-network clients still need a superbill with your NPI, tax ID, Rhode Island license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Rhode Island superbills
Independent counseling in Rhode Island is commonly written as LMHC. Also print the letters you actually hold: LMHC, LICSW, LMFT, Licensed Psychologist. The board to match your number against is the Rhode Island Board of Mental Health Counselors and Marriage and Family Therapists. License line hint: RI LMHC #.
Rhode Island counselors use LMHC. Independent social workers use LICSW.
Telehealth coding (national rules, used in Rhode Island)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Rhode Island
Professional counseling is generally exempt from Rhode Island sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Enacted (confirm if privileges are live)
- PSYPACT (psychologists): Active
- Social Work Compact: Enacted (confirm if privileges are live)
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
LPC or LMHC in Rhode Island?
LMHC. Use the title on your CIIS license.
What to put on a Rhode Island superbill
- Your legal name, LMHC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Rhode Island license number as printed by the Rhode Island Board of Mental Health Counselors and Marriage and Family Therapists
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Rhode Island private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Rhode Island license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select Rhode Island defaults when your rendering authority and the client's location for the session are Rhode Island (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
Rhode Island counselors use Licensed Mental Health Counselor (LMHC)—not LPC. Independent social workers commonly use LICSW. MFTs use LMFT; psychologists use Licensed Psychologist. Print the exact title on your RI Department of Health license.
Other titles commonly used on Rhode Island superbills:
- LICSW — print the exact letters on your Rhode Island license/board record.
- LMFT — print the exact letters on your Rhode Island license/board record.
- Licensed Psychologist — print the exact letters on your Rhode Island license/board record.
Exact credential dropdown strings:
`LMHC` · `LICSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Counselor and MFT licenses are issued under Rhode Island Department of Health board structures. Verify via RIDOH / MyLicense verification and copy the number exactly.
Official verification: https://healthri.mylicense.com/Verification/. Copy the license number exactly as shown. The generator hint RI LMHC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — enacted (not yet operational for privileges). Rhode Island appears on counselingcompact.gov/map/compact-states. Privileges are usable only when both home and remote states are live/operational—check the current live list before relying on a privilege.
- PSYPACT — yes. Rhode Island participates for psychologists who hold required ASPPB authorizations (E.Passport / TAP as applicable). Confirm on psypact.gov. PSYPACT does not authorize counselor, social work, or MFT practice.
- Social Work Licensure Compact — enacted. Rhode Island is listed on swcompact.org/compact-jurisdictions. Multistate social work licenses are not yet being issued while the commission implements its data system.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A Rhode Island license, a live Counseling Compact privilege naming Rhode Island (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Rhode Island sales tax generally applies to tangible personal property and certain taxable services. Professional counseling and psychotherapy session fees are generally exempt. Leave tax at $0 on clinical invoices. Confirm with the Rhode Island Division of Taxation if you sell goods.

Two worked Rhode Island examples
Rhode Island's paperwork runs through the Department of Health's MyLicense portal, and its credential alphabet follows the New England pattern: LMHC for counselors, LICSW for independent clinical social workers — never LPC. The two documents below come from a fictional Providence practice: one private-pay invoice, one out-of-network superbill. All identifiers are placeholders.
Example 1 — an in-office private-pay Rhode Island invoice
An LMHC sees a self-pay client for weekly 45-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| May 8 | Individual psychotherapy, 45 min | 90834 | 11 | $160.00 | $160.00 |
| May 15 | Individual psychotherapy, 45 min | 90834 | 11 | $160.00 | $160.00 |
| May 22 | Individual psychotherapy, 45 min | 90834 | 11 | $160.00 | $160.00 |
| May 29 | Individual psychotherapy, 45 min | 90834 | 11 | $160.00 | $160.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $640.00 | $640.00 | |||
POS 11 is accurate for office sessions. The tax line stays at $0.00 because professional counseling is generally exempt from Rhode Island sales tax — therapy-only invoices carry no taxable line. The credential is LMHC, and the license field follows the RI LMHC # hint — copy the number exactly as the RIDOH MyLicense verification displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Warwick has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Aug 6 | 90791 | 95 | 10 | F41.1 | $190.00 | $190.00 |
| Aug 13 | 90834 | 95 | 10 | F41.1 | $160.00 | $160.00 |
| Aug 20 | 90834 | 95 | 10 | F41.1 | $160.00 | $160.00 |
| Aug 27 | 90834 | 95 | 10 | F41.1 | $160.00 | $160.00 |
| Total | $670.00 | $670.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous telemedicine for the plans that require it. The intake bills 90791; the 45-minute follow-ups bill 90834.
The provider block carries the rendering name with LMHC, the license number copied verbatim from healthri.mylicense.com, NPI, practice address, and tax ID. If the rendering clinician were an independent clinical social worker, the credential would be LICSW. The paid column is populated because OON reimbursement repays actual expenditures.
Both parties were in Rhode Island, so the state license sufficed. Rhode Island has enacted the Counseling Compact — privileges require both home and remote states to be live on the Commission map before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.
Common mistakes on Rhode Island superbills
- Writing "LPC" instead of "LMHC". Rhode Island's counseling credential is Licensed Mental Health Counselor. LPC reads as an out-of-state license and fails to match the RIDOH record.
- Printing LCSW when the license says LICSW. Rhode Island's independent clinical social work credential is LICSW — print the exact letters on the Department of Health license.
- Printing a supervised status as LMHC. Associate and provisional statuses are not the independent title — print the exact credential held and identify the supervisor when the plan asks.
- Treating enacted compact status as live. Rhode Island is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Adding a sales-tax line. Professional counseling is generally exempt from Rhode Island sales tax. Leave the line at $0 unless you sell taxable goods.

Why these sources matter
Every factual claim on this page traces back to a primary source: Rhode Island Board of Mental Health Counselors and Marriage and Family Therapists, the official interstate-compact commissions, and Rhode Island's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
