Maryland superbill and invoice for LCPC, LCSW, LMFT, and psychologists

This page is the same Therapist Invoice Generator generator with Maryland selected. Out-of-network clients still need a superbill with your NPI, tax ID, Maryland license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.

License titles used on Maryland superbills

Independent counseling in Maryland is commonly written as LCPC. Also print the letters you actually hold: LGPC, LCPC, LCSW-C, LCMFT, Licensed Psychologist. The board to match your number against is the Maryland Board of Professional Counselors and Therapists. License line hint: MD LCPC #.

Maryland independent counselors are LCPCs. Clinical social workers use LCSW-C. Candidates: LGPC.

Telehealth coding (national rules, used in Maryland)

In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.

Sales tax on therapy in Maryland

Professional psychotherapy is generally exempt from Maryland sales tax. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).

Interstate compacts (verify before you practice)

  • Counseling Compact: Enacted (confirm if privileges are live)
  • PSYPACT (psychologists): Active
  • Social Work Compact: Enacted (confirm if privileges are live)

Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.

LCSW or LCSW-C in Maryland?

The clinical independent social work license is LCSW-C. Use that string on the rendering-provider line.

What to put on a Maryland superbill

  • Your legal name, LCPC or other credential, practice address, phone
  • NPI (Type 1) and EIN or SSN as tax ID
  • Maryland license number as printed by the Maryland Board of Professional Counselors and Therapists
  • Client name, date of birth, member ID if they want reimbursement
  • Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid

Orientation only — not billing, tax, or legal advice. Payer rules beat this page.

Maryland private-practice clinicians issue invoices for cash-pay clients and superbills so clients can file out-of-network (OON) claims. This generator creates either PDF in the browser with Maryland defaults: LCPC as the independent counseling title, LGPC for graduate professional counselors, LCSW-C for clinical social workers, and fields for NPI, CPT, ICD-10, place of service, and payments. The tool is browser-only—no PHI upload.

Use Maryland defaults when your Maryland authority and the client’s location for the session authorize care (or another lawful pathway you hold). Print the exact credential on your Board of Professional Counselors and Therapists verification. This page is documentation support, not legal or billing advice.

License titles that belong on the form

Maryland independent counselors are Licensed Clinical Professional Counselors (LCPC). Candidates/graduate-level counselors commonly hold Licensed Graduate Professional Counselor (LGPC). Do not print LCPC during the graduate/candidate period.

Other titles commonly used on Maryland superbills:

  • LCSW-C — Licensed Certified Social Worker – Clinical (Maryland’s clinical independent social work license—not “LCSW” alone when the clinical string is LCSW-C).
  • LCMFT — Licensed Clinical Marriage and Family Therapist (use the exact board abbreviation on your certificate).
  • Licensed Psychologist — psychology license under Maryland psychology regulation.

Exact credential dropdown strings:

`LGPC` · `LCPC` · `LCSW-C` · `LCMFT` · `Licensed Psychologist`

Match the Board’s public verification record letter-for-letter. If you hold additional specialty certifications, note them in the notes field rather than altering the primary credential line.

Board, license number, lookup

Professional counselors and related therapy licenses under this board are regulated by the Maryland Board of Professional Counselors and Therapists. Online license verification is available through the Board’s PCT Portal verify tool (primary-source style data updated from the licensing database).

Copy the license number exactly as the portal shows it. The generator hint MD LCPC # is a counselor cue; LCSW-C, LCMFT, and psychologist licensees paste their own Maryland numbers.

Include Type 1 NPI (Type 2 for groups), tax ID, and practice address. Taxonomy belongs in notes or electronic claims when requested.

Interstate practice: Counseling Compact / PSYPACT / SW compact

Counseling Compact — enacted (not yet operational for privileges in Maryland). Maryland appears on the Counseling Compact jurisdictions list. Enactment is not live privilege issuance. Confirm Maryland’s operational status and the remote state’s live status on counselingcompact.gov before interstate episodes. Associates/graduate credentials are not compact-eligible.

PSYPACT — yes. Maryland participates for psychologists with required ASPPB authorizations. PSYPACT does not authorize LCPC, LCSW-C, or LCMFT practice.

Social Work Licensure Compact — enacted (not yet operational for multistate licenses). Maryland appears on the official Social Work Compact jurisdictions list. Multistate licenses are not yet available—verify at swcompact.org.

Telehealth + place of service

Bill telehealth with POS 10 (patient at home) or POS 02 (elsewhere) and modifier 95 when the payer requires synchronous telemedicine coding. Do not use POS 11 for video-only sessions.

Interstate telehealth requires Maryland licensure, a live Counseling Compact privilege when available, PSYPACT for psychologists, or another lawful pathway. Print only credentials that authorize that encounter.

Sales tax on therapy invoices

Maryland sales and use tax applies to tangible personal property and to specifically defined taxable services. Comptroller materials state that charges for many personal and professional services are not part of taxable price when they are not defined taxable services and any sale of tangible property is inconsequential. Ordinary psychotherapy and counseling session fees are therefore generally billed with sales tax at $0.

If you separately sell taxable digital products, taxable telecommunications, or other listed taxable services, follow Comptroller rules for those lines. Confirm edge cases with the Comptroller of Maryland or a tax professional.

Documentation tips for Maryland OON claims

Maryland’s LGPC vs LCPC distinction matters for both board compliance and payer credentialing. Graduate professional counselors should keep supervisor identification available for superbills when plans require it, and should never substitute LCPC on paper during the LGPC period.

Clinical social workers should prefer LCSW-C over informal “LCSW” shorthand when that is the license type on the Board of Social Work Examiners record—payers in Maryland often key on the “-C” clinical designation.

When Maryland becomes operational in the Counseling Compact (if/when the Commission adds it to the live list), LCPCs who live in Maryland would apply through CompactConnect like other live states; until then, interstate counseling into or from Maryland requires traditional licensure pathways.

Two worked Maryland examples

Maryland's credential alphabet is denser than most states': LCPC for independent counselors, LGPC for the graduate tier, LCSW-C for clinical social workers, LCMFT for marriage and family therapists — and a payer matching the letters against the Board of Professional Counselors and Therapists record will catch a wrong one. The two documents below come from a fictional Baltimore practice. All identifiers are placeholders.

Example 1 — an in-office private-pay Maryland invoice

An LCPC sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.

Date of serviceServiceCPTPOSChargePaid
Apr 1Individual psychotherapy, 60 min9083711$170.00$170.00
Apr 8Individual psychotherapy, 60 min9083711$170.00$170.00
Apr 15Individual psychotherapy, 60 min9083711$170.00$170.00
Apr 22Individual psychotherapy, 60 min9083711$170.00$170.00
Sales tax (see below)$0.00
Total$680.00$680.00

POS 11 is accurate for office sessions. The tax line stays at $0.00 because Maryland generally exempts personal and professional services that are not defined taxable services — psychotherapy session fees are typically outside the base per Comptroller guidance. The credential is LCPC, Maryland's independent clinical counseling title, and the license field follows the MD LCPC # hint — copy the number exactly as the Board's PCT Portal verify tool displays it.

Example 2 — a telehealth out-of-network superbill

A second client in Annapolis has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth coding, and the complete provider identity block.

Date of serviceCPTModifierPOSICD-10ChargePaid
Aug 4907919510F43.10$200.00$200.00
Aug 11908379510F43.10$170.00$170.00
Aug 18908379510F43.10$170.00$170.00
Aug 25908379510F43.10$170.00$170.00
Total$710.00$710.00

The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 flags synchronous audio-video service for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.

The provider block carries the rendering name with LCPC, the license number copied verbatim from the PCT Portal, NPI, practice address, and tax ID. Specialty certifications belong in the notes field — the credential line itself matches the board record letter-for-letter. The paid column is populated because OON reimbursement repays actual expenditures.

Both parties were in Maryland, so the state license sufficed. Maryland has enacted the Counseling Compact — privileges require both home and remote states to be live before reliance — and participates in PSYPACT for psychologists holding ASPPB authorizations.

Common mistakes on Maryland superbills

  • Printing LCPC while holding LGPC. Licensed Graduate Professional Counselors hold the supervised tier. Disclose LGPC and identify the supervisor when the plan asks — do not print the independent clinical title early.
  • Writing "LCSW" when the license says LCSW-C. Maryland's clinical independent social work license is LCSW-C — the "-C" is part of the title, not decoration. Print the exact board abbreviation.
  • Truncating the MFT credential. Maryland's title is LCMFT — Licensed Clinical Marriage and Family Therapist. Match the certificate exactly.
  • Treating enacted compact status as live. Maryland is a Counseling Compact member, but privileges require operational status on both ends — verify counselingcompact.gov first.
  • Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
  • Adding a sales-tax line. Psychotherapy session fees are typically outside Maryland's taxable-services base. Leave tax at $0 unless you sell taxable goods.

Why these sources matter

Every factual claim on this page traces back to a primary source: Maryland Board of Professional Counselors and Therapists, the official interstate-compact commissions, and Maryland's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
