Hawaii superbill and invoice for LMHC, LCSW, LMFT, and psychologists
This page is the same Therapist Invoice Generator generator with Hawaii selected. Out-of-network clients still need a superbill with your NPI, tax ID, Hawaii license number, CPT codes, ICD-10 diagnoses, place of service, and fees. Private-pay clients usually need a simpler invoice — toggle the document type above.
License titles used on Hawaii superbills
Independent counseling in Hawaii is commonly written as LMHC. Also print the letters you actually hold: LMHC, LCSW, LMFT, Licensed Psychologist. The board to match your number against is the Hawaii DCCA Professional & Vocational Licensing. License line hint: HI MHC #.
Hawaii uses LMHC. The state is outside PSYPACT and the Counseling Compact, so telehealth into Hawaii usually needs a Hawaii license.
Telehealth coding (national rules, used in Hawaii)
In-office sessions use place of service 11 and no telehealth modifier. Video visits with the client at home use POS 10 and modifier 95. If the client is not at home, use POS 02. Audio-only, when the payer allows it, is modifier 93. Do not pair POS 11 with modifier 95.
Sales tax on therapy in Hawaii
Hawaii's GET can apply to a wide range of services. Confirm with a Hawaii tax professional whether your specific psychotherapy receipts are exempt. This generator defaults tax to zero. Add a note, not a tax engine, if your situation is unusual (retail products, groups billed as events, etc.).
Interstate compacts (verify before you practice)
- Counseling Compact: Not a member
- PSYPACT (psychologists): Not a member
- Social Work Compact: Not a member
Enacted is not the same as operational. A compact privilege does not replace documenting the license or privilege you used on the date of service.
Is Hawaii therapy taxed?
Hawaii taxes many services under the General Excise Tax. Licensed health care has specific treatment — do not assume mainland “services are exempt” rules. Get local advice before adding tax to an invoice.
What to put on a Hawaii superbill
- Your legal name, LMHC or other credential, practice address, phone
- NPI (Type 1) and EIN or SSN as tax ID
- Hawaii license number as printed by the Hawaii DCCA Professional & Vocational Licensing
- Client name, date of birth, member ID if they want reimbursement
- Each date of service, POS, CPT (90834 / 90837 / 90791 are the usual set), ICD-10, fee, amount paid
Orientation only — not billing, tax, or legal advice. Payer rules beat this page.
Hawaii private-practice clinicians use superbills for out-of-network (OON) insurance reimbursement and invoices for self-pay clients. This generator builds either document in the browser—no PHI upload—and downloads a PDF with rendering credential, Hawaii license number, NPI, CPT, ICD-10, place of service, and payment lines.
Select Hawaii defaults when your rendering authority and the client's location for the session are Hawaii (or another jurisdiction you are lawfully authorized to serve). Match the exact title on your board record. This page is workflow documentation only—not legal, billing, or tax advice.
License titles that belong on the form
Hawaii's counseling credential is Licensed Mental Health Counselor (LMHC). Print LMHC (or the exact MHC string on your certificate)—not LPC. Social workers, MFTs, and psychologists use their Hawaii titles as licensed.
Other titles commonly used on Hawaii superbills:
- LCSW — print the exact letters on your Hawaii license/board record.
- LMFT — print the exact letters on your Hawaii license/board record.
- Licensed Psychologist — print the exact letters on your Hawaii license/board record.
Exact credential dropdown strings:
`LMHC` · `LCSW` · `LMFT` · `Licensed Psychologist`
Independently licensed clinicians list their own license only. Associates and provisionally licensed clinicians must use the associate/provisional title and should identify the supervisor when a payer asks. Do not put the independent title on the form during a supervised period.
Board, license number, lookup
Mental health counselor licenses are administered by Hawaii DCCA Professional & Vocational Licensing. Verify through the eHawaii PVL license search and enter the number exactly.
Official verification: https://pvl.ehawaii.gov/pvlsearch/. Copy the license number exactly as shown. The generator hint HI MHC # is a placeholder—replace it with your real board number. Include individual NPI (and group NPI if applicable) plus EIN/SSN as the payer requires.
Interstate practice: Counseling Compact / PSYPACT / SW compact
Snapshot for 2026-09-14 (always re-check official maps):
- Counseling Compact — no. Hawaii is not listed as a member jurisdiction on counselingcompact.gov/map/compact-states. Do not rely on compact privileges to counsel clients located in Hawaii.
- PSYPACT — no. Hawaii is not an active PSYPACT jurisdiction on the official map as of this retrieval. Psychologists generally need a Hawaii license (or another lawful route) to practice into Hawaii.
- Social Work Licensure Compact — no at retrieval (Hawaii not listed on swcompact.org/compact-jurisdictions). Do not advertise multistate SW practice via the compact until the official map includes Hawaii.
Verify before you rely on it. Compact membership and privilege systems change. This page's flags are for generator defaults only.
Telehealth + place of service
For video psychotherapy superbills, use the place-of-service codes payers expect:
- POS 10 — patient at home
- POS 02 — telehealth other than home
- Modifier 95 — when the plan wants synchronous telemedicine marked
Do not code a video-only session as POS 11. A Hawaii license, a live Counseling Compact privilege naming Hawaii (when operational), or another lawful authorization must cover the client's location. Put your licensed practice address on the form even when the session is remote.
Sales tax on therapy invoices
Hawaii imposes a General Excise Tax (GET) on many business activities, including services, unless a specific exemption applies (tax.hawaii.gov/get; Exemptions-GET). GET is a tax on the business; some providers pass cost through to clients. Certain Medicare/Medicaid/TRICARE-related healthcare exemptions exist for listed provider types—confirm whether your license category qualifies. Default the invoice tax line to $0 until a Hawaii tax professional tells you how to display any pass-through; file GET returns as required.
How to fill the Hawaii generator
- Choose Superbill or Invoice.
- Set jurisdiction to HI.
- Enter rendering name; select LMHC (default) or another credential you currently hold from the dropdown.
- Complete practice demographics, NPI, tax ID, and Hawaii board license number.
- Add client member ID / DOB only if needed for the OON claim.
- Enter DOS, CPT, ICD-10, units, fees, and payments.
- Telehealth: POS 10 or 02; modifier 95 when required.

Two worked Hawaii examples
Hawaii paperwork differs from the mainland pattern in ways that matter: the counseling credential is LMHC, verification runs through DCCA's Professional & Vocational Licensing search, and the tax question needs its own handling — Hawaii's General Excise Tax is levied on the business rather than charged to the client as a sales tax. The two documents below come from a fictional Honolulu practice. All identifiers are placeholders.
Example 1 — an in-office private-pay Hawaii invoice
An LMHC sees a self-pay client for weekly 60-minute sessions in the office. No insurer is involved, so the document is an invoice — services, dates, charges, payments — and the diagnosis column can stay empty.
| Date of service | Service | CPT | POS | Charge | Paid |
|---|---|---|---|---|---|
| Apr 1 | Individual psychotherapy, 60 min | 90837 | 11 | $190.00 | $190.00 |
| Apr 8 | Individual psychotherapy, 60 min | 90837 | 11 | $190.00 | $190.00 |
| Apr 15 | Individual psychotherapy, 60 min | 90837 | 11 | $190.00 | $190.00 |
| Apr 22 | Individual psychotherapy, 60 min | 90837 | 11 | $190.00 | $190.00 |
| Sales tax (see below) | $0.00 | ||||
| Total | $760.00 | $760.00 | |||
POS 11 is accurate for office sessions. The tax line reads $0.00, but for a different reason than on the mainland: Hawaii's General Excise Tax is imposed on the business's gross receipts, not charged to the client as a sales tax. That means you leave the invoice line at $0 while still owing GET on those receipts and filing GET returns as required. Some providers pass the cost through to clients — do that only once a Hawaii tax professional has told you how to display it, and never improvise a percentage. Certain healthcare exemptions exist for listed provider types, so confirm whether your license category qualifies. The credential is LMHC, and the license field follows the HI MHC # hint — copy the number exactly as the eHawaii PVL search displays it.
Example 2 — a telehealth out-of-network superbill
A second client in Hilo has a PPO and attends weekly by video from home. The document must satisfy an adjuster's checklist, so it becomes a superbill: the invoice fields plus diagnosis, telehealth place of service, the telemedicine modifier, and the complete provider identity block.
| Date of service | CPT | Modifier | POS | ICD-10 | Charge | Paid |
|---|---|---|---|---|---|---|
| Jun 4 | 90791 | 95 | 10 | F32.1 | $215.00 | $215.00 |
| Jun 11 | 90837 | 95 | 10 | F32.1 | $190.00 | $190.00 |
| Jun 18 | 90837 | 95 | 10 | F32.1 | $190.00 | $190.00 |
| Jun 25 | 90837 | 95 | 10 | F32.1 | $190.00 | $190.00 |
| Total | $785.00 | $785.00 | ||||
The client joined from home, so POS 10 applies — telehealth from anywhere else would take 02, and 11 would falsely claim an office visit. Modifier 95 marks synchronous audio-video service for the plans that require it. The intake bills 90791; the 60-minute follow-ups bill 90837.
The provider block carries the rendering name with LMHC — never LPC, which reads as an out-of-state credential here — plus the DCCA PVL license number, NPI, practice address, and tax ID. The paid column is populated because OON reimbursement repays actual expenditures.
Both parties were in Hawaii throughout, which kept the licensing question clean — and clean is what you want, because Hawaii sits outside every interstate compact pathway relevant here. Hawaii is not a Counseling Compact member and does not participate in PSYPACT, so no privilege or authorization substitutes for a Hawaii credential covering the client's location.
Common mistakes on Hawaii superbills
- Treating the $0 invoice line as meaning no tax is owed. Hawaii's General Excise Tax is imposed on your gross receipts, not collected from the client like a sales tax. The invoice can show $0 while you still owe GET and must file returns — and any pass-through belongs on the invoice only on a Hawaii tax professional's advice.
- Writing "LPC" instead of "LMHC". Hawaii's counseling credential is Licensed Mental Health Counselor. LPC strings fail to match DCCA records and can stall verification.
- Assuming compact or PSYPACT coverage. Hawaii is outside the Counseling Compact and does not participate in PSYPACT — a client located in Hawaii needs a Hawaii license or another lawful pathway.
- Guessing the license number. Copy the exact string from the eHawaii PVL search rather than reconstructing a format — the payer verifies against that record.
- Coding telehealth as POS 11. Use 10 for a client at home and 02 for telehealth elsewhere. POS 11 describes an office visit.
- Leaving the paid column blank. OON plans reimburse money already spent — a superbill without payments usually comes back unanswered.
FAQ
Is Hawaii therapy taxed?
Hawaii taxes many services under GET. Licensed health care has specific treatment—do not assume mainland services-are-exempt rules. Get local advice before adding tax to an invoice.
Is Hawaii in the Counseling Compact?
No—Hawaii is outside the Counseling Compact. Telehealth or in-person practice into Hawaii generally requires a Hawaii license (or another lawful authorization).
Should my Hawaii invoice show GET?
GET is imposed on the business; some providers pass it through. Confirm with a Hawaii tax professional and DOT guidance before displaying a tax line.
Can a Hawaii psychologist use PSYPACT?
No—Hawaii is outside active PSYPACT participation as of this retrieval. Confirm any bill status on psypact.gov; enactment proposals are not the same as active membership.
What do OON payers want on a Hawaii superbill?
Name + Hawaii credential (LMHC or as licensed), board license number, NPI, DOS, CPT, ICD-10, POS, charges, payments, and telehealth modifiers when required.
Why these sources matter
Every factual claim on this page traces back to a primary source: Hawaii DCCA Professional & Vocational Licensing — Mental Health Counselor Program, the official interstate-compact commissions, and Hawaii's own tax authority. That matters because out-of-network billing guidance ages badly — license titles get renamed, compacts add members, and taxable-service lists get amended. Secondhand summaries lag those changes by months. The links below are the records themselves, with the date each was checked, so you can re-verify anything here before you rely on it for a claim.
